摘　　要：根据仝小林提出的"郁热虚损"理论,糖尿病周围神经病变属于"虚、损"阶段,络病范畴,其中最为顽固难治的痛性糖尿病周围神经病变总以气虚血瘀为基本病机,气血虚弱为本,气滞血瘀为标,涉及寒入骨髓,表现为阳虚寒凝血瘀。辨治过程中首控血糖,糖络并治,扶正补虚,助阳透寒。临床应用过程中要"糖络并治"贯全程,扶正补虚要求"衡"。多选用黄芪桂枝五物汤为基本方,寒凝痛显者则加乌头汤。According to the theory of "depression,heat,deficiency,damage" proposed by Professor Tong Xiaolin,diabetic peripheral neuropathy belongs to the stage of "deficiency and damage" and the category of collateral diseases.Painful diabetic peripheral neuropathy,the most stubborn and refractory one,always has qi deficiency and blood stasis as its basic pathogenesis,qi and blood weakness as its root,qi stagnation and blood stasis as its manifestations,involved with cold invasion into the bone marrow,manifested as yang deficiency,cold coagulation and blood stasis.In the process of its differentiation and treatment,the first step is to control blood sugar,with combined treatment of sugar and collaterals,so as to strengthen vital qi and tonify deficiency,reinforce yang and eliminate cold.In clinical application,"combined treatment of sugar and collaterals" must be carried out throughout the whole process,and "strengthening vital qi and tonifying deficiency" must be kept in balance.Huangqi Guizhi Wuwu Decoction is mostly selected as the basic prescription,and Wutou Decoction may be added for those with cold coagulation and obvious pain.
