加味一贯煎治疗肝肾阴虚型肝纤维化

摘　　要：目的观察加味一贯煎治疗慢性乙型肝炎肝纤维化肝肾阴虚证的临床疗效。方法选择60例慢性乙型肝炎肝纤维化肝肾阴虚证患者,随机分成观察组和对照组,各30例。对照组予恩替卡韦治疗,观察组在对照组的基础上服用加味一贯煎,均治疗6个月。观察2组中医证候评分、ALT、AST、HBV-DNA、肝纤维化相关指标(肝纤四项,APRI评分和FIB指数)。结果治疗组有效率(93.3%,28/30)高于对照组(40%,12/30)(P<0.05)。2组治疗前后中医证候积分比较,差异具有统计学意义(P<0.05),且治疗组优于对照组(P<0.05)。2组治疗前后乙肝DNA、ALT、AST比较,差异具有统计学意义(P<0.05),且治疗组优于对照组(P<0.05)。2组治疗前后肝纤四项,APRI评分和FIB指数比较,差异具有统计学意义(P<0.05),且治疗组优于对照组(P<0.05)。结论加味一贯煎可以有效改善慢性乙型肝炎肝纤维化肝肾阴虚证的临床相关指标和症状。Objective To observe the clinical effect of Jiawei Yiguan Decoction in treating chronic hepatitis B liver fibrosis with syndrome of yin deficiency in liver and kidney.Methods Sixty patients with chronic hepatitis B liver fibrosis with syndrome of liver-kidney yin deficiency were randomly divided into observation group and control group,30 cases in each group.The control group was treated with Entecavir,and the observation group was administered Jiawei Yiguan Decoction on the basis of the control group.They were both treated for 6 months.The two groups’ TCM syndrome scores,ALT,AST,HBV-DNA,and liver fibrosis related indicators (four items of liver fiber,APRI score and FIB index) were observed and analyzed.Results The effective rate of the treatment group (93.3%,28/30) was higher than that of the control group (40%,12/30) (P<0.05).Comparing the scores of TCM syndromes before and after treatment in the two groups,the differences were statistically significant (P<0.05),and the treatment group was better than the control group (P<0.05).The differences of hepatitis B DNA,ALT,AST before and after treatment between the two groups were statistically significant (P<0.05),and the treatment group was better than the control group (P<0.05).The four items of liver fiber in the two groups before and after treatment,the APRI score and the FIB index were compared,and the differences were statistically significant (P< 0.05),and the treatment group was better than the control group (P<0.05).Conclusion Jiawei Yiguan Decoction can effectively improve the clinical indicators and symptoms of chronic hepatitis B liver fibrosis with liver-kidney yin deficiency syndrome.
