慢性肾功能衰竭患者经升清降浊胶囊治疗前后血清Hcy、FGF-23水平变化及其与疗效的关系

摘　　要：目的探讨慢性肾功能衰竭(CRF)患者经升清降浊胶囊治疗前后血清同型半胱氨酸(Hcy)、成纤维细胞生长因子(FGF-23)水平变化及其与疗效的关系。方法选择 122例慢性肾功能衰竭患者作为研究对象,并选择同期50名健康体检人员作为健康对照组。依据升清降浊胶囊治疗效果分为有效组(92例)和无效组(30例)。结果慢性肾功能衰竭患者治疗前后的血清Hcy和FGF-23水平均显著高于健康对照者(P<0.05)。治疗后,有效组的血清Hcy、FGF-23水平相比治疗前显著下降(P<0.05),但无效组的血清Hcy、FGF-23水平相比于治疗前差异无统计学意义(P>0.05);治疗后,有效组的血清Hcy、FGF-23水平显著低于无效组患者(P<0.05)。Logistic 回归分析结果显示,高水平血清Hcy、FGF-23是慢性肾功能衰竭患者经升清降浊胶囊治疗无效的独立影响因素(P<0.001)。结论检测血清Hcy、FGF-23水平变化可作为评估慢性肾功能衰竭患者经升清降浊胶囊治疗后疗效有效性的重要指标。Objective To investigate the changes of serum homocysteine (Hcy) and fibroblast growth factor (FGF-23) levels before and after treatment with Shengqing Jiangzhuo Capsules in patients with chronic renal failure (CRF) and their relationship with therapeutic effect.Methods A total of 122 CRF patients admitted to our hospital from January 2017 to December 2018 were selected as the research subjects,and 50 healthy persons after examination during the same period were selected as the healthy control group.Serum Hcy and FGF-23 levels were measured by enzyme-linked immunosorbent assay.According to the therapeutic effect of Shengqing Jiangzhuo Capsules,the subjects were divided into effective group (92 cases) and ineffective group (30 cases).Results The serum Hcy and FGF-23 levels were significantly higher in CRF patients before and after treatment than in healthy control group (P<0.05).After treatment,the serum Hcy and FGF-23 levels of the effective group were significantly lower than before treatment (P<0.05),but the serum Hcy and FGF-23 levels of the ineffective group were not significantly different from those before treatment (P>0.05).After treatment,serum Hcy and FGF-23 levels in the effective group were significantly lower than those in the ineffective group (P<0.05).Logistic regression analysis showed that high levels of serum Hcy and FGF-23 were independent factors from the treatment of CRF patients with Shengqing Jiangzhuo Capsules (P<0.001).Conclusion Detecting changes in serum Hcy and FGF-23 levels can be used as an important index for evaluating the efficacy of Shengqing Jiangzhuo Capsules in the treatment of patients with chronic renal failure.It is worthy of clinical promotion.
