PCNE分类体系在心内科药物相关问题中的探索与实践

摘　　要：目的探索欧洲药学监护网(PCNE)分类体系在心内科临床药师实际工作中的应用及实际影响。方法2017年9月至2018年6月,专职临床药师全程参与临床工作,通过对所在病区进行全医嘱审核等方式发现潜在的药物相关问题(DRPs)并进行干预,然后借助PCNE分类体系进行统计分析。结果在研究期间,专职临床药师发现136例患者在住院治疗期间中存在不同程度的药物相关问题,总计165例;实际干预159例,干预成功率为96.9%。经分析发现,“药物治疗效果不佳”(81例,49.1%)及“药物治疗引起/可能引起潜在的药物不良反应”(47例,28.5%)为临床DRPs的最常见问题,而“患者治疗地点转移”(75例,45.4%)、“剂量的选择”(33例,20.0%)及“药物的选择”(23例,13.9%)为问题发生的最主要原因。冠心病、心律失常、高血压与糖尿病为该心内科DRPs的高发疾病。结论PCNE分类体系的引入能够提升临床药师发现和解决DRPs的能力,明确DRPs的人群及疾病分布特点,对制定规范化的临床药师干预流程、标准化的临床药师工作记录方式、系统化的分析DRPs的原因及结果具有重要意义。Objective To explore the application and influence of European Pharmaceutical Care network(PCNE)classification system for cardiology clinical pharmacists.Methods The full-time clinical pharmacists were involved in the whole clinical process.Potential DRPs were detected by reviewing the all medcial orders in the ward.The intervention and statistical analysis was carried out with the help of PCNE classification system.Results During this study,165 DRPs were found in 136 patients,159 intervention,the success rate of intervention was 96.9%.The most common DRP was"effect of drug treatment not optimal"(81 cases,49.1%)and"adverse drug event(possibly)occuring"(47 cases,28.5%),and the most common DRP category was"patient transfer related"(75 cases,45.4%),"does selection"(33 cases,20.0%)and"drug selection"(23 cases,13.9%).Coronary heart disease,arrhythmia,hypertension and diabetes were the high incidence of DRPs in this department.Conclusion It is of great practical significance to identify the population distribution characteristics of DRPs,standard of process for clinical pharmacists,and systematic analysis of DRPs by use of PCNE classification system.
