不稳定型心绞痛患者血浆H-FABP、cTnI及差异蛋白表达及其意义

摘　　要：目的观察不稳定型心绞痛(UAP)患者血浆心型脂肪酸结合蛋白(H-FABP)、心肌钙蛋白I(cTnI)及差异蛋白的表达并分析其意义。方法选取郑州市第七人民医院收治的UAP患者与稳定型心绞痛(SAP)患者各90例,分为UAP组与SAP组,并取同期90例健康体检者作为对照组,比较3组的一般临床资料与相关危险因素、血浆H-FABP、cTnI及差异蛋白-免疫球蛋白κ链C结构域(IGKC)水平,并比较UAP患者不同冠脉狭窄程度亚组、预后良好组与不良组血浆H-FABP、cTnI、IGKC水平。结果血浆H-FABP、cTnI水平:UAP组>SAP组>对照组,差异有统计学意义(P<0.05);血浆IGKC蛋白水平:UAP组>对照组>SAP组,差异有统计学意义(P<0.05);轻度组与中度组血浆H-FABP、cTnI、IGKC蛋白水平明显低于重度组与完全闭塞组(P<0.05);预后良好组血浆H-FABP、cTnI、IGKC水平低于不良组(P<0.05)。结论UAP患者存在血浆H-FABP、cTnI、IGKC表达异常升高现象,其水平检测对UAP患者病情判定、预后评估具有重要指导意义。【Objective】To observe the expression of plasma heart-type fatty acid binding protein(H-FABP),cardiac troponin I(cTnI)and differential proteins in patients with unstable angina pectoris(UAP)and analyze its significance.【Methods】Ninety patients with UAP and 90 patients with stable angina pectoris(SAP)treated in the hospital were selected and included in UAP group and SAP group,respectively.Ninety healthy people completing physical examination in the same period were selected as the control group.The general clinical data,related risk factors,levels of plasma H-FABP,cTnI and differential protein-immunoglobulin kappa chain C domain(IGKC)were compared among the three groups.The levels of plasma H-FABP,cTnI and IGKC in patients with different degrees of coronary stenosis,in the good prognosis group and the poor prognosis group were compared.【Results】The levels of plasma H-FABP and cTnI were the highest in UAP group,followed by SAP group and control group(P<0.05).The level of plasma IGKC protein was the highest in UAP group,followed by control group and SAP group(P<0.05).The levels of plasma HFABP,cTnI and IGKC protein in mild group and moderate group were significantly lower than those in severe group and complete occlusion group(P<0.05).The levels of plasma H-FABP,cTnI and IGKC in good prognosis group were significantly lower than those in poor prognosis group(P<0.05).【Conclusion】Plasma H-FABP,cTnI and IGKC levels are abnormally high in patients with UAP.The detection of above indexes is of important guiding significance for condition judgement and prognosis evaluation of UAP.
