摘  要:目的探究肺动脉环缩术(PAB)应用于≥6月龄的先天性心脏病合并肺动脉高压患儿的效果。方法选择近年郑州大学附属儿童医院收治的107例先天性心脏病合并肺动脉高压患儿的临床资料。按年龄分组分为<6月龄组(45例),≥6月龄组(62例),均采用PAB手术治疗,对比两组SPAP、SpO2等指标,≥6月龄组中术前用药与未用药患儿及Ⅱ期矫治患儿的SPAP、SpO2变化,并记录术后死亡情况。结果<6月龄组和≥6月龄组术后SPAP、SpO2、呼吸机使用时间、ICU停留时间比较差异无统计学意义(P>0.05)。≥6月龄组中术前用药、术前未用药患儿在PAB术后的SPAP、SpO2比较差异无统计学意义(P>0.05)。<6月龄组死亡率为2.22%,≥6月龄组死亡率为3.23%(P>0.05)。Ⅱ期矫治患儿中单心室矫治和双心室矫治患儿的SPAP、SpO2差异无统计学意义(P>0.05)。结论对≥6月龄患儿在术前通过给氧、药物靶向治疗以降低SPAP,为PAB手术治疗提供机会,不影响术后肺动脉压力及饱和度,且可对Ⅱ期矫治手术提供术前保障。【Objective】To explore the effect of pulmonary artery circumcision(PAB)in children with congenital heart disease and pulmonary hypertension.【Methods】The clinical data of 107 children with congenital heart disease and pulmonary hypertension treated in our hospital in recent years were selected.According to the age group,the patients were divided into two groups:the group of<6 months old(45 cases)and the group of≥6 months old(62 cases).All patients were treated with PAB.The changes of SPAP and SpO2 were compared between the two groups.The changes of SPAP and SpO2 in the group of≥6 months old were recorded.【Results】There was no significant difference between the<6 months old group and the≥6 months old group in SPAP,SpO2,ventilator use time and ICU stay time(P>0.05).In≥6 months old group,there was no significant difference in SPAP,SpO2 after PAB between children with preoperative medication and without preoperative medication(P<0.05).The mortality rate in the group of<6 months old was 2.22%,and that in the group of≥6 months old was 3.23%(P>0.05).There was no significant difference in SPAP,SpO2 between children treated with single-ventricle repair and children treated with biventricular repair(P>0.05).【Conclusion】In order to reduce SPAP,the patients aged≥6 months were given oxygen and drug targeted therapy before operation,which provided opportunity for PAB operation,did not affect the pressure and saturation of pulmonary artery after operation,and provided preoperative guarantee for the second stage correction operation.
