克氏针固定治疗第一掌骨基底部骨折的疗效及对患者第一腕掌关节的稳定性及拇指功能的影响

摘　　要：目的:探讨对第一掌骨基底部骨折疾病,使用克氏针固定的手术治疗方式的效果及对患者第一腕掌关节稳定性、拇指功能康复的价值。方法:回顾性分析60例第一掌骨基底部骨折的临床手术资料,患者均采取克氏针固定治疗,随访患者6个月,评价患者临床疗效及术后第一腕掌关节稳定性及拇指功能。结果:60例患者经治疗的优良率为96.67%,术后未见严重并发症;同术后1d时刻相比较,术后6个月患者第一腕掌关节稳定性与拇指功能优良率分别为98.33%、96.67%,明显高于术后1 d 86.67%、83.33%的优良率(P<0.05)。结论:采取克氏针固定治疗第一掌骨基底部骨折具有良好效果,且可有效促进第一腕掌关节稳定性及拇指功能,值得推广。Objective: To investigate the effect of surgical treatment with kirschner wire fixation on the fracture of the base of the first metacarpal bone and the value of rehabilitation of the first carpometacarpal joint. Methods: The clinical operation data of60 cases of first metacarpal base fracture were analyzed retrospectively. All the patients were treated with kirschner’s needle fixation. The patients were followed up for 6 months to evaluate the clinical effect, stability of the first carpal joint and thumb function.Results: The excellent and good rate of 60 patients was 96.67%. Compared with the 1 st day after surgery, the excellent and good rates of the first carpometacarpal joint stability and thumb function were 98.33% and 96.67%, respectively. 6 months after surgery,the rates were 86.67% and 83.33% one day after surgery(P<0.05). Conclusion: Kirschner’s needle fixation for the treatment of the fracture of the base of the first metacarpal bone has a good effect, and can effectively promote the stability of the first carpometacarpal joint and the function of thumb. So it is worthy of promotion.
