摘  要:目的探索社区青中年人群血清谷氨酰转肽酶浓度与动脉硬化的关系。方法本研究是一个横断面研究,所有年龄18~65岁的参与者均于2014年1月至2016年12月来自广东社区,且均采集人口信息学及完善体格检查、血糖、血脂、肝酶和脉搏波传导速度检测,根据肱踝脉搏波传导速度将入选者分为动脉硬化(≥1 400 cm/s)组和正常组(<1 400 cm/s)。采用多变量逻辑回归、分层和交互作用分析入选人群谷氨酰转肽酶浓度与动脉硬化的关系。结果共入选865人,其中男性和动脉硬化人数分别为54(26.60%)和367(42.43%)例,平均年龄、谷氨酰转肽酶浓度与肱踝脉搏波传导速度分别为(50.11±8.67)岁,(25.38±24.09)IU/L和(1 398.72±221.43)cm/s。动脉硬化组人群谷氨酰转肽酶浓度明显高于正常组,差异有统计学意义[(29.00±26.80)IU/L vs.(22.71±21.52)IU/L,P<0.001]。当谷氨酰转肽酶作为连续变量时,在调整年龄、吸烟、肾功能、性别、血脂、饮酒、体质量指数、收缩压、空腹血糖、原发性高血压(高血压)和糖尿病后,谷氨酰转肽酶与动脉硬化密切相关(OR=1.34,95%CI:1.05~1.71,P=0.0202);当谷氨酰转肽酶作为分类变量时,以第一分位作为对照,第二、三和四分位与动脉硬化的关系分别为OR=1.67(95%CI:0.99~2.83,P=0.0560)、OR=2.14(95%CI:1.23~3.73,P=0.0068)和OR=2.31(95%CI:1.29~4.15,P=0.0050)(趋势P=0.006)。各亚组中也表明谷氨酰转肽酶与动脉硬化密切相关,且与各亚组变量无交互作用。结论青中年人群中血清谷氨酰转肽酶浓度与肱踝脉搏波传导速度密切关系,是动脉硬化的独立危险因素。Objectives To study the relationship between serum concentration of glutamyl transpeptidase and atherosclerosis in young adults and middle-aged population.Methods This was a cross-sectional study.All participants aged 18-65 and were enrolled from Guangdong community between January 2014 to December 2016.Collected data included demographics,data from physical examination,blood glucose,blood lipids,liver enzymes and pulse wave conduction velocity detection.The participants were classified into atherosclerosis group(≥1400 cm/s)and normal group(<1400 cm/s)according to pulse wave velocity of the iliac crest.Multivariate Logistic regression with stratified and interactive analyses were used to analyze the relationship between serum concentration of glutamyl transpeptidase and atherosclerosis.Results A total of 865 patients were enrolled.Number of male participants and participants with atherosclerosis were 54(26.60%)and 367(42.43%),respectively.The mean age,serum concentration of glutamyl transpeptidase and sputum pulse wave conduction velocity were(50.11±8.67)years,(25.38±24.09)IU/L and(1398.72±221.43)cm/s.Serum concentration of glutamine transpeptidase was significantly higher in patients with atherosclerotic group than in normal group[(29.00±26.80)IU/L vs.(22.71±21.52)IU/L,P<0.001].When glutamyl transpeptidase was treated as a continuous variable,glutamyl transpeptidase significantly associated with atherosclerosis(OR=1.34,95%CI:1.05-1.71,P=0.0202),after adjusting for age,gender,systolic blood pressure,body mass index,fasting blood glucose,blood lipids,renal function,smoking,alcohol consumption,hypertension,and diabetes.When glutamyl transpeptidase was treated as a categorical variable,using the first quintile as referent,the risk of atherosclerosis for participants in the second,third and fourth quartiles were OR=1.67(95%CI:0.99-2.83,P=0.0560),OR=2.14(95%CI:1.23-3.73,P=0.0068)and OR=2.31(95%CI:1.29-4.15,P=0.0050)(trend P=0.006).Serum concentration of glutamyl transpeptidase closely related to atherosclerosis
