摘  要:目的探讨慢性心力衰竭(chronic heart failure,CHF)患者血清微小RNA-24(micro RNA-24,miR-24)、转化生长因子-β1(transforming growth factor-β1,TGF-β1)表达水平及其与心肌重构和心功能的相关性。方法选取2017年2月至2018年9月深圳市中西医结合医院CHF患者108例为研究对象,并根据纽约心脏协会(New York Heart Association,NYHA)心功能分级标准分为Ⅰ~Ⅱ级(36例)、Ⅲ级(35例)、Ⅳ级(37例);另选取同期健康志愿者56名为对照组。所有受试者均进行超声心动图检查;采用实时定量聚合酶链反应(quantitative real-time polymerase chain reaction,qRT-PCR)检测血清miR-24、TGF-β1 mRNA表达水平。采用Pearson法分析miR-24与TGF-β1的相关性及其与心肌重构指标[左心室舒张末期内径(left ventricular end-diastolic diameter,LVEDD)、左心室收缩末期内径(left ventricular end-systolic diameter,LVESD)、左心房内径(left atrial diameter,LAD)、左心室后壁厚度(left ventricular posterior wall depth,LVPWD)、左心室质量指数(left ventricular mass index,LVMI)、左心室重构指数(left ventricular remodeling index,LVRI)],心功能指标[心排血量(cardiac output,CO)、左心室射血分数(left ventricular ejection fraction,LVEF)]的关系。结果 CHF组不同心功能等级患者血清miR-24表达水平及LVRI、CO、LVEF均明显低于对照组(P<0.05),Ⅲ级与Ⅳ级组患者明显低于Ⅰ~Ⅱ级组(P<0.05),且Ⅳ级组患者明显低于Ⅲ级组(P<0.05),差异均有统计学意义;CHF组不同心功能等级患者血清TGF-β1 mRNA表达水平及LVEDD、LAD、LVPW、LVMI均明显高于对照组(P<0.05),Ⅲ级与Ⅳ级组患者明显高于Ⅰ-Ⅱ级组(P<0.05),且Ⅳ级组患者明显高于Ⅲ级组(P<0.05),差异均有统计学意义。Pearson分析显示miR-24与TGF-β1、LVEDD、LVPW、LVMI均呈负相关(P<0.05),而与LVRI、CO、LVEF均呈正相关(P<0.05);TGF-B1与LVEDD、LVPW、LVMI均呈正相关(P<0.05),而与CO、LVEF均呈负相关(P<0.05)。结论 CHF患者血清miR-24Objectives To investigate the serum concentrations of micro RNA-24(miR-24)and transforming growth factor-β1(TGF-β1)in patients with chronic heart failure(CHF)and their correlations with myocardial remodeling and cardiac function.Methods Totally 108 patients with CHF in General Hospital of the Second People′s Hospital(Group)of Baoan District from February 2017 to September 2018 were selected as the study subjects,and according to New York Heart Association(NYHA)classification criteria,they were classified into gradeⅠ-Ⅲ(36 cases),gradeⅢ(35 cases)and gradeⅣ(37 cases);another 56 healthy volunteers in the same period were selected as control group.All the subjects were examined by echocardiography.Quantitative real-time polymerase chain reaction(qRTPCR)was used to detect the serum concentrations of miR-24 and TGF-β1.Pearson method was used to analyze the correlation between miR-24 and TGF-β1 and their relationships with left ventricular end-diastolic diameter(LVEDD),left atrial diameter(LAD),left ventricular posterior wall depth(LVPWD),left ventricular mass index(LVMI),left ventricular remodeling index(LVRI),cardiac output(CO),left ventricular ejection fraction(LVEF).Results Serum concentration of miR-24,LVRI,CO and LVEF in CHF group were significantly lower than those in control group(P<0.05),and in gradeⅢandⅣgroups were significantly lower than those in gradeⅠ-Ⅱgroup(P<0.05),and in gradeⅣgroup were significantly lower than those in gradeⅢgroup(P<0.05).Serum concentration of TGF-β1 mRNA,LVEDD,LAD,LVPWD and LVMI in CHF group were significantly higher than those in control group(P<0.05),and in gradeⅢandⅣgroups were significantly higher than those in gradeⅠ-Ⅱgroup(P<0.05),and in gradeⅣgroup were significantly higher than those in gradeⅢgroup(P<0.05).Pearson analysis showed that miR-24 negatively correlated with TGF-β1,LVEDD,LVPW,LVMI(P<0.05),and positively correlated with LVRI,CO,LVEF(P<0.05).TGF-β1 positively correlated with LVEDD,LVPWD,LVMI(P<0.05),but negatively correlated wit
