摘　　要：目的观察CO_(2)点阵激光联合卤米松乳膏治疗烧伤后增生性瘢痕的临床效果。方法选择我院2016年5月至2019年12月门诊收治的265例烧伤后增生性瘢痕患者为研究对象,将其随机分为治疗组(135例,208处瘢痕)和对照组(130例,184处瘢痕)。对照组单纯使用CO_(2)点阵激光治疗,治疗组采用CO_(2)点阵激光联合卤米松乳膏外涂治疗。比较两组患者的临床疗效及治疗前、后的温哥华瘢痕量表(VSS)评分。结果治疗组的治疗总有效率为97.12%,高于对照组的86.41%,差异具有统计学意义(P<0.05)。治疗前,两组的VSS评分比较,差异无统计学意义(P>0.05);治疗3、5次后,两组患者的VSS评分均较治疗前显著降低,且治疗组低于对照组,差异具有统计学意义(P<0.05)。结论CO_(2)点阵激光联合卤米松乳膏外涂治疗烧伤后增生性瘢痕的临床效果显著,可明显改善瘢痕的色泽、柔软度、厚度、血管分布情况。Objective To observe the clinical effect of CO_(2) fractional laser combined with halometasone cream in the treatment of hypertrophic scar after burn.Methods A total of 265 patients with hypertrophic scar after burn admitted in our hospital from May 2016 to December 2019 were selected as the research objects and randomly divided into treatment group(135 cases,208 scars)and control group(130 cases,184 scars).The control group was only treated with CO_(2) fractional laser,and the treatment group was treated with CO_(2) fractional laser combined with external halometasone cream.The clinical efficacy,Vancouver scar scale(VSS)scores before and after treatment were compared between the two groups.Results The total effective rate of treatment in the treatment group was 97.12%,which was higher than 86.41%in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the VSS score between the two groups(P>0.05);after 3 and 5 times of treatment,the VSS scores in the two groups were significantly lower than those before treatment,those in the treatment group were lower than the control group,and the differences were statistically significant(P<0.05).Conclusion The CO_(2) fractional laser combined with halometasone cream in the treatment of hypertrophic scar after burn has a significant clinical effect,which can significantly improve the color,softness,thickness and vascular distribution of scar.
