摘 要:目的系统评价生命早期益生菌干预对预防儿童过敏性鼻炎的疗效。方法计算机检索知网、万方、PubMed、Cochrane library、Embase、Web of science数据库,搜索有关孕期和/或生命早期益生菌干预预防儿童过敏性鼻炎疗效的随机对照试验,检索起止时间为2007年1月至2019年9月。由评价员筛选文献、提取资料并评价纳入研究的偏倚风险后,采用Stata 11.0软件进行Meta分析。结果最终纳入13篇文献,干预组和对照组病例分别为2005例和2161例。合并效应量结果显示,生命早期益生菌干预并不能预防儿童过敏性鼻炎的发生风险(RR为1.00,95%CI:0.83–1.22,P=0.971),以研究地区进行亚组分析显示,研究地区为大洋洲干预组对预防儿童过敏性鼻炎效果显著(RR为0.82,95%CI:0.69-0.96,Z=2.44,P=0.015)。结论生命早期益生菌干预在预防过敏性鼻炎方面的无显著差异。但就目前而言,由于纳入研究样本量小,随机对照试验较少,本结论尚需高质量、大样本的研究予以证实。Objective To systematically evaluate the effect of probiotic supplementation during pregnancy and/or infants on the prevention of allergic rhinitis in children.Methods Wanfang,pubMed,cochrane library,embase,web of science databases were searched for randomized controlled trials on the efficacy of probiotics during pregnancy and/or infants in the prevention of allergic rhinitis.The retrieval period was from January 2007 to September 2019.After the two authors independently screened the literature,extracted the data,and assessed the risk of bias in the included studies,Meta analysis was performed with Stata 12.0 software.Results Thirteen articles were finally included.The intervention group and the control group were 2005 and 2161 respectively.The combined effect size results showed that the use of probiotics during pregnancy and/or infants does not reduce the incidence of allergic rhinitis in children(RR=1.00,95%CI:0.83–1.22,P=0.971).Subgroup analysis in the study area showed that the study area is the Oceania intervention group,which has a significant effect on the prevention of allergic rhinitis(RR=0.82,95%CI:0.69-0.96,Z=2.44,P=0.015).Conclusion Probiotic supplementation during pregnancy and/or infants has no significant difference in preventing allergic rhinitis.However,at present,due to the small sample size of the included studies and the small number of randomized controlled trials,This conclusion needs to be confirmed by high quality,large sample studies.
