摘  要:目的研究微量元素六项检测水平与丙型肝炎病毒核糖核酸(HCV-RNA)载量的相关性。方法选取2018年5月一2020年5月在舟山市普陀区人民医院收治的丙型肝炎病毒(HCV)感染患者70例为HCV感染组,另选取同期体检的70名健康志愿者为对照组,比较两组入院时微量元素钙(Ca),锌(Zn),铜(Cu),铅(Pb),铁(Fe),镁(Mg)水平,根据HCV-RNA定量检测结果将HCV感染组分为阳性(n=48)和阴性(n=22)两个亚组,比较其Ca、Zn、Cu、Pb、Fe、Mg水平差异,利用受试者工作特征曲线(ROC)分析Ca、Zn、Cu、Fe对HCV感染患者HCV-RNA阳性的预测评估价值Pearson相关性分析HCV-RNA定量与微量元素的相关性。结果HCV感染组Ca、Zn水平显著低于对照组,Cu、Fe水平显著高于对照组(P<0.05);HCV-RNA定量显著高于对照组(P<0.05);HCV感染患者依据HCV-RNA定量分为阳性亚组48例和阴性亚组22例,阳性亚组Ca、Zn水平显著低于阴性亚组,Cu、Fe水平显著高于阴性亚组(P<0.05);ROC曲线结果显示,Ca、Zn、Cu、Fe均具有一定的预测评估价值,AUC为(0.574~0.704);Pearson相关分析结果,HCV-RNA定量与Ca、Zn水平呈负相关,与Cu、Fe水平呈正相关(P<0.05)。结论HCV感染患者Ca、Zn、Cu、Fe水平呈现异常表达,HCV-RNA定量与Ca、Zn、Cu、Fe微量元素的表达密切相关,但与Pb、Mg的表达无相关性。OBJECTIVE To observe the correlation between the detection of six trace elements and the load of hepatitis C virus RNA(HCV-RNA). METHODS Totally 70 patients with HCV infection who were treated in the Putuo District People′s Hospital from May 2018 to May 2020 were assigned as the HCV infection group, meanwhile, 70 healthy people who received physical examination were set as the control group.The levels of calcium(CA), zinc(Zn), copper(Cu), lead(PB), iron(FE) and magnesium(mg) were compared between the two groups at admission.The patients of the HCV infection group were divided into the positive group with 48 cases and the negative group with 22 cases according to the result of HCV-RNA quantitative detection, the differences in the levels of Ca, Zn, Cu, Pb, Fe and Mg were observed, the values of Ca, Zn, Cu and Fe in prediction and assessment of positive HCV-RNA were analyzed by means of ROC curve, and Pearson correlation analysis was performed for the correlation between the quantification of HCV-RNA and the trace elements. RESULTS The levels of Ca and Zn of the HCV infection group were significantly lower than those of the control group, while the levels of Cu and Fe of the HCV infection group were significantly higher than those of the control group(P<0.05);the HCV-RNA quantification of the HCV infection group was significantly higher than that of the control group(P<0.05).The patients with HCV infection were divided into the positive subgroup with 48 cases and the negative subgroup with 22 cases according to the quantification of HCV-RNA, the levels of Ca and Zn of the positive subgroup were significantly lower than those of the negative subgroup group, and the levels of Cu and Fe of the positive subgroup were significantly higher than those of the negative subgroup(P<0.05).ROC curve analysis indicated that Ca, Zn, Cu and Fe had certain predictive value, with the AUC ranging between 0.574 and 0.704.
