摘　　要：目的探讨应用乳腺癌超声图像特征Logistic回归方程预测腋窝淋巴结转移风险的研究。方法采用回顾性分析收集本院因早期浸润性乳腺癌行乳腺切除的患者,从本院数据报告系统获取患者的超声图像,提取超声特征后建立影像组学评分公式,并建立Logistic回归模型进行分析。结果300例患者发生ALN转移118例,未发生ALN转移182例。乳腺癌超声特征在良恶性组间的比较均具有显著性差异(P<0.05),而异物、导管异常、复杂囊肿、乳内淋巴结、簇状小囊肿和皮肤肿物等6个征象在良恶性组间的比较则均无显著性差异(P>0.05);建立的Logistic回归模型判断患者是否具有腋窝淋巴结转移的风险灵敏度为84.4%、特异度95.3%、准确度91.3%、曲线下面积为0.962、标准误为0.005,P<0.0001,95%置信区间(95%CI)为(0.955、0.972)。结论应用乳腺癌超声图像特征Logistic回归方程预测腋窝淋巴结转移风险做出有效评估。Objective To explore the application of the logistic regression equation of breast cancer ultrasound image characteristics to predict the risk of axillary lymph node metastasis.Methods Retrospective analysis was used to collect the patients who underwent mastectomy due to early invasive breast cancer from in our hospital.The ultrasound images of the patients were obtained from the data reporting system of our hospital.After the ultrasound features were extracted,the imaging score formula was established,and the logistic regression model was established for analysis.Results There were 118 cases of ALN metastasis in 300 patients and 182 cases without AlN metastasis.There was significant difference in the ultrasound characteristics of breast cancer between the benign and malignant groups(P<0.05),while there was no significant difference in the six signs of foreign body,catheter abnormality,complex cyst,intramammary lymph node,clustered small cyst and skin tumor between the benign and malignant groups(P>0.05);the established logistic regression model to determine whether the patients had the risk sensitivity of axillary lymph node metastasis(P>0.05)It was 84.4%,specificity 95.3%,accuracy 91.3%,area under the curve 0.962,standard error 0.005,P<0.0001,95%confidence interval(95%CI)0.955,0.972.Conclusions The sonographic features of breast cancer based on the image histology can be used to assess the risk of AlN metastasis of breast cancer noninvasively.
