摘 要:目的探讨分层应变技术评价糖尿病及糖耐量异常患者左室收缩功能及同步性的临床应用价值。方法选取糖耐量异常52例、糖尿病60例、100例健康志愿者作为研究对象,应用分层应变技术测定左室心内膜下纵向应变(LPS_(Endo))、中层纵向应变(LPS_(Mid))及心外膜下心肌纵向应变(LPS_(Epi))、左室整体收缩期纵向应变(GLPS)、应变达峰时间离散度(PSD)。结果(1)对照组、糖耐量异常组、糖尿病组心肌GLPS绝对值逐渐降低,两两相比有统计学意义(P<0.05)。对照组与糖耐量异常组LPS_(Endo)差异有统计学意义(P<0.05)。糖尿病组LPS_(Endo)、LPS_(Mid)、LPS_(Epi)与糖耐量异常组、对照组差异均有统计学意义(P<0.05)。(2)对照组、糖耐量异常组及糖尿病组PSD值依次增加,两两比较均有统计学意义(P<0.05)。PSD与GLPS绝对值呈负相关(r=-0.755,P=0.000)。结论分层应变技术可早期定量评价糖尿病及糖耐量异常患者心肌受损改变,PSD可早期评估糖尿病及糖耐量异常患者左心室收缩的同步性,为临床提供可靠诊断依据。Objective To investigate the clinical value of layer-specific strain technique in evaluating left ventricular systolic function and synchronization in diabeties and impaired glucose tolerence.Methods 52 patients of impaired glucose tolerance,60 patients of diabetes and 100 health volunteers were enrolled.The myocardical layer-specific strain was used to measure the LPS of the left ventricular myocardium of subendocardium,the middle layer,the subepicardium,and myocardial strain and the PSD of the whole myocardial layers.Results(1)The LPS gradually decreased from the control group to the diabetes group,and the statistical signicance was observed on the LPS among three groups(P<0.05).Compared with the control group,LPS_(Endo) in the impaired glucose tolerance group was statistically significant difference in the control group(P<0.05).The LPS in the subendocardial,middle,epicardial of the diabetes group were all reduced and significantly different in the control group and the glucose tolerance group(P<0.05).(2)There were statistically significant differences on the PSD among three groups(P<0.05).There were negative correlation between absolute value of GLPS and PSD(r=-0.755,P=0.000).Conclusions The layer-specific strain can quantitively evaluate the changes of systolic function on myocardial globle and layers longitudinal strain in patients with diabetes and impaired glucose tolerance,PSD for evaluating primary synchronous in left ventricular myocardial contraction in patients with diabeties and impaired glucose tolerence has certain advantages.
