摘　　要：目的探讨超声引导下穿刺活检对肝移植后肺部病变的诊断价值。方法回顾性分析因肝移植术后肺部病变行超声引导下穿刺活检的19例患者资料,对超声图像资料、穿刺取材成功率、诊断价值及并发症进行总结分析。结果19例肝移植术后肺部病变的位置均位于肺脏周围,超声图像可以清晰显示。穿刺取材成功率为100%(19/19),18例有明确病理诊断(18/19,94.7%),其中肺部感染9例(炎性病变6例、曲霉菌2例、新型隐球菌1例)、肺转移癌5例、肺结核4例;1例无明确病理诊断(1/19,5.3%)。术后均无并发症发生。结论超声引导下肺穿刺活检术是诊断肝移植后肺部病变性质的安全有效手段。Objective To evaluate the diagnostic value of ultrasound-guided needle biopsy in pulmonary lesions after liver transplantation.Methods Data of 19 patients who underwent ultrasound-guided needle biopsy for pulmonary lesions after liver transplantation were retrospectively analyzed.Ultrasonic image data,success rate of puncture sampling,diagnostic value and complications were summarized and analyzed.Results The location of pulmonary lesions after liver transplantation in all the 19 cases was located around the lung,and the ultrasound image could be clearly shown.The success rate of puncture sampling was 100%(19/19),and 18 cases had definite pathological diagnosis(18/19,94.7%),including 9 cases of pulmonary infection(6 cases of inflammatory lesions,2 cases of aspergillus,1 case of cryptococcus neoformans),5 cases of lung metastasis,and 4 cases of tuberculosis.1 case had no definite pathological diagnosis(1/19,5.3%).The incidence of postoperative complications was 0%(0/19).Conclusions Ultrasound-guided lung biopsy is a safe and effective method to diagnose the nature of lung lesions after liver transplantation.
