摘　　要：目的探讨儿童四肢长骨骨嗜酸性肉芽肿的超声诊断及分型表现。方法回顾性分析经病理证实的37例骨嗜酸性肉芽肿的临床、超声及放射学资料。结果病灶37例,均为单发。25例可见骨质破坏,12例未见骨质破坏;16例骨内可见低回声肿物;9例骨内呈低回声区;7例骨内无肿物,但周围软组织有影像学改变;3例骨内无肿物,周围软组织亦无明显影像改变;2例骨内有肿物,但超声无法显示。其中有2例出现骨膜反应,有3例低回声肿物中出现浑浊积液,并有1例出现液液平面,有5例低回声肿物中出现强回声。超声诊断与病理符合率达到22%(8/37)。超声诊断与X线诊断符合率达到68%(25/37)。结论熟悉本病的超声表现分型,结合X线表现及临床资料,有助于本病的诊断。Objective To explore the ultrasonic diagnosis and classification of eosinophilic granuloma of long bone in children.Methods The clinical,ultrasonic and radiological data of 37 cases of eosinophilic granuloma of bone confirmed by pathology were analyzed retrospectively.Results There were 37 cases of lesions,all of which were single.Bone destruction was seen in 25 cases,no bone destruction in 12 cases,hypoechoic mass in 16 cases,hypoechoic area in 9 cases,no tumor in bone but imaging changes in surrounding soft tissue in 7 cases,no mass in bone in 3 cases.There were no obvious imaging changes in the surrounding soft tissue,and there were intraosseous masses in 2 cases,but ultrasound could not show them.Periosteal reaction was found in 2 cases,turbid effusion in 3 cases,liquid-liquid level in 1 case,and strong echo in 5 cases.The coincidence rate between ultrasound diagnosis and pathology was 22%(8/37).The coincidence rate of ultrasonic diagnosis and X-ray diagnosis was 68%(25/37).Conclusions Familiarity with the ultrasonic classification of the disease,combined with X-ray findings and clinical data,is helpful to the diagnosis of the disease.
