摘 要:尽管细针穿刺细胞学检查(fine needle aspiration cytology,FNAC)在甲状腺结节诊断中得到广泛应用,但依然有标本不满意或细胞学结果不确定等缺点。超声引导下针穿活检(core needllebiopsy,CNB)已被认为是一种诊断甲状腺结节安全.有效的替代方法。目前,已建立用于甲状腺CNB结果分类的病理报告系统。CNB的诊断价值已在越来越多的研究中得到验证,即CNB可以解决FNAC无法诊断或标本不满意等问题,提高细胞学结果不确定甲状腺结节的确诊率,进而提高甲状腺结节诊断准确率。CNB并发症轻微,严重并发症发生率较低,大部分患者耐受良好,相对安全。Although fine needle aspiration cytology has been widely used in the diagnosis of thyroidnodule,there are still some limitations,including high rate nondiagnostic or unsatisfactory cytology and un-certain cytological results.Ultrasound-guided core needle biopsy(CNB)has been regarded as a safe and ef-fective alternation.Currently,a pathology reporting system of thyroid core needle biopsy has been estab-lished for reporting thyroid CNB results.More studies have identified the diagnostic value of CNB since it re-duced the rate of nondiagnostic/unsatisfactory or indeterminate cytology,therefore improved the diagnosis ac-curacy of thyroid nodule.Meanwhile,accumulated evidence has demonstrated that CNB is well tolerated andrelatively safe for most patients due to its low rate of major and minor complications.
