摘 要:目的研究血清25-羟基维生素D[25(OH)D]水平与慢性阻塞性肺疾病急性加重期(AEC0PD)临床分级的相关性。方法收集2017年11月至2019年10月就诊我院急诊科的AECOPD患者(观察组)及同期呼吸科门诊就诊的COPD稳定期(对照组)患者各120例的临床资料,入组后24小时内测定两组的血清25(OH)D水平。采用临床分级将AECOPD患者分为3组。结果AECOPD患者血清25(OH)D水平明显低于稳定期COPD患者(P<0.05);25(OH)D缺乏与AECOPD分级呈正相关(P<0.05);多因素logistic回归显示hs-CRP、25(OH)D缺乏是AECOPD的独立影响因素。结论25(OH)D缺乏与AECOPD的临床分级相关,早期检测血清25(OH)D可能改善AECOPD患者的危险分层。Objective To study the correlation between 25-hydroxyvitamin D deficiency and different grades of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 120 patients diagnosed with AECOPD in the emergency department of our hospital were recruited from Nov 1,2017 to Oct 1,2019.Meanwhile,another 120 COPD patients at stable stage visited in respiratory clinic were sampled.Patients with AECOPD were divided into three groups according to the clinical classification standard of AECOPD.The levels of 25(OH)D were measured in all patients.Results The level of 25(OH)D in the case group was significantly lower than that in the control group(P<0.05).The deficiency of 25(OH)D was positively correlated with the level of AECOPD grades(P<0.05).Multivariate logistic regression showed that hypersensitive C-reactive protein(hs-CRP)and 25(OH)D were independent factors of AECOPD.Conclusion The deficiency of 25(OH)D is related to the clinical grades of AECOPD.Early detection of serum 25(OH)D can improve the risk stratification of patients with AECOPD.
