喉镜下声带手术中应用右美托咪定的临床麻醉有效性

摘　　要：目的:评定喉镜下声带手术中应用右美托咪定的麻醉有效性。方法:2018年4月-2020年5月收治行喉镜下声带手术治疗的患者62例,随机分为两组,各31例。试验组采取右美托咪定麻醉;对照组采取丙泊酚麻醉。比较两组不同时段收缩压、舒张压、心率水平及阿托品和乌拉地尔的使用情况。结果:两组麻醉前、插管时、置入喉镜时及拔管时收缩压、舒张压水平比较,差异无统计学意义(P>0.05);两组麻醉前、拔管时心率水平比较,差异无统计学意义(P>0.05);试验组插管时心率水平高于对照组,置入喉镜时心率水平低于对照组,差异均有统计学意义(P<0.05)。试验组阿托品和乌拉地尔的使用率低于对照组,差异有统计学意义(P<0.05)。结论:在喉镜下声带手术治疗中,选择右美托咪定的麻醉有效性较佳。Objective:To explore the clinical anesthetic efficacy of dexmedetomidine in vocal cord surgery under laryngoscope.Methods:From April 2018 to May 2020,62 patients undergoing vocal cord surgery under laryngoscope were selected and randomly divided into two groups with 31 cases in each group.The experimental group was anesthetized with dexmedetomidine and the control group was anesthetized with propofol.We compared the systolic blood pressure,diastolic blood pressure,heart rate at different times and the use of atropine and urapidil.Results:There was no significant difference in systolic and diastolic blood pressure between the two groups at before anesthesia,intubation,laryngoscope insertion and extubation(P>0.05).There was no significant difference in heart rate between the two groups before anesthesia and during extubation(P>0.05).In the experimental group,the heart rate during intubation was higher than that in the control group,and the heart rate level at the time of laryngoscope insertion was lower than that of the control group,the difference was statistically significant(P<0.05).In the experimental group,the use rate of atropine and urapidil was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion:The clinical anesthetic efficacy of dexmedetomidine in vocal cord surgery under laryngoscope is better.
