摘　　要：目的通过循证医学方法评价和规范吗替麦考酚酯的超说明用药。方法回顾性抽取2019年吗替麦考酚酯住院医嘱423份,对超说明书用药情况进行统计分析,并以Micromedex Thomson分级标准、国内外权威指南、专家共识、诊疗规范或高级别文献研究为依据,判断超说明书用药的合理性。结果吗替麦考酚酯超说明书用药医嘱281份,占含吗替麦考酚酯总医嘱的66.4%,均为超适应证用药。发生率排名前3的科室依次为肾内科、风湿免疫科和神经科。超适应证医嘱中临床诊断以肾病综合征、系统性红斑狼疮和视神经脊髓炎多见。结论吗替麦考酚酯超适应证用药有国内外诊疗指南推荐或高级别文献支持,患者用药有指征,用药合理。AIM To evaluate and standardize the off-lable drug use of mycophenolate mofetil by evidence-based medicine. METHODS A retrospective survey of 423 hospitalized cases for mycophenolate mofetil in 2019 was conducted, statistical analysis of off-lable use, according to Micromedex Thomson classification standards, authoritative guidelines at home and abroad, expert consensus, diagnosis and treatment standards or high-level literature research, to judge the rationality of the off-lable use. RESULTS There were 281 cases of off-lable use, with a proportion of 66.4% of all cases for mycophenolate mofetil. The top 3 departments with incidence were the Departments of Nephrology, Rheumatology and Neurology. The clinical diagnosis of off lable drug use was more common in nephrotic syndrome, systemic lupus erythematosus and optic neuromyelitis. CONCLUSION The mycophenolate superindicator is recommended by domestic and foreign diagnostic guidelines or supported by high-level literature. The patients are indicated for the medication and the medication is reasonable.
