带袢钢板治疗急性Rockwood Ⅲ型肩锁关节脱位的疗效观察

摘　　要：目的:比较带袢钢板与锁骨钩钢板治疗RookwoodⅢ型肩锁关节脱位的临床效果。方法:回顾性分析我院2016年5月-2018年5月收治的50例急性RockwoodⅢ型肩锁关节脱位患者进行手术治疗,其中25例行带袢钢板内固定(观察组),25例行锁骨钩钢板内固定(对照组),2组患者术后平均随访12个月,比较2组患者的术后疼痛、手术相关指标、临床治疗总有效率及术后并发症发生情况。结果:术后观察组lazzcano肩关节功能评分优良率为%%,显著高于对照组的88%(P<0.05);观察组患者术后疼痛方面明显优于对照组(P<0.05);2组手术时间比较差异无统计学意义(P>0.50);观察组切口长度、术中出血量均明显小于对照组(P<0.05);观察组患者的术后并发症总发生率为12%,低于对照组的24%,差异有统计学意义(P<0.05)。结论:带袢钢板内固定治疗急性Rockwoodffl型肩锁关节脱位符合生物力学及解剖学要求,具有创伤小、疗效确切、不需要再次取出内固定物等优点,临床效果优于锁骨钓钢板,值得临床推广。Objective:To compare the clinical ffect of treatment of Rockwood type Ⅲ acromioclavicular joint dislocation with loop plate and clavicular hook plate.Methods:50 patients with acute Rockwood type Ⅲ acromioclavicular joint dislocation admitted to our hospital from May 2016 to may 2018 were analyzed retrospectively.Among them,25 patients were treated with loop plate internal fixation(group A)and 25 patients were treated with clavicular hook plate internal fixation(group B).The patients in the two groups were fol-lowed up for an average of 12 months.The postoperative pain,operation related indexes,clinical treatment fficiency and operation of the two groups were compared Post complications.Results:The excellent and good rate of lazzcanos shoulder joint function score in group A was 96%,signifcantly higher than that in group B(P<0.05);the pain in the loop plate group was significantly better than that in the clavicular hook plate group(P<0.05);there was no significant diference in the operation time between the two groups(P>0.50);the incision length and intraoperative bleeding in the loop plate group were significantly lower than that in the clavicular hook plate group(P<0.05);the postoperative complications in the observation group were sigificantly lower than that in the clavicular hook plate group(P<0.05)The total ineidence of the disease was 12%,lower than 24%of the control group,the difference was statistically significant(P<0.05).Conclusion:The treatment of acute Rockwood type Ⅲ acromioclavicular joint dislocation with loop plate internal fixation meets the requirements of biomechanics and anatomy,has the advantages of small trauma,exact ffct,no need to take out the intermal fixation again,and the clinical effect is better than that of clavicular hook plate,which is worthy of clinical promotion.
