摘 要:目的了解尘肺病患者体质指数(BMI)与肺功能的基本情况,探讨两者之间的相关性。方法对167例尘肺病患者进行身高、体质量、肺功能检查,分析BMI与肺功能的关系。结果随着尘肺病期别提高,BMI、肺功能第1秒用力呼气量(FEV_(1))、1秒率(FEV_(1)%)、呼气峰流速值(PEF)、最大通气量(MVV)、一氧化碳弥散量(DLCO)各指标有降低趋势(r=-0.202、-0.458、-0.494、-0.496、-0.261、-0.414,P<0.01),残气/肺总量(RV/TLC)指标呈上升趋势(r=0.223,P<0.01)。随着年龄增长,肺功能FEV_(1)、FEV_(1)%、PEF、MVV、DLCO各指标呈降低趋势(r=-0.221、-0.194、-0.179、-0.185、-0.196,P<0.05),RV/TLC指标呈上升趋势(r=0.262,P<0.01),BMI与年龄无关。BMI、肺功能与工龄不相关。将患者按照BMI分成3组,低BMI组(BMI≤18.4kg/m^(2))5例,正常BMI组(BMI 18.5~24.99kg/m^(2))111例,高BMI组(BMI>25kg/m^(2))51例。随着BMI值增高,肺功能FEV_(1)%、PEF、DLCO各指标有升高趋势(r=0.186、0.181、0.264,P<0.05),RV/TLC指标呈下降趋势(r=-0.178,P<0.05)。低BMI组的FEV_(1)、PEF、MVV、DLCO指标值低于正常BMI组和高BMI组,RV/TLC指标值高于其他两组(P<0.01)。结论BMI与肺功能有显著相关性,BMI高者肺功能较好,有助于评估尘肺病病情的严重程度。Objective To understand the basic situation of body mass index(BMI)and pulmonary function in pneumoconiosis patients,and to explore the correlation between them.Methods 167patients with pneumoconiosis were examined for their height,weight and lung function.Results With the increase of the pneumoconiosis stages,BMI,pulmonary function FEV_(1),FEV_(1),PEF,MVV,DLCO%each index had a tendency to reduction(r=0.202,0.458,0.494,0.496,0.261,0.414,P<0.01),RV/TLC target is on the rise(r=0.223,P<0.01).As the increase of the age,pulmonary function FEV_(1),FEV_(1),PEF,MVV,DLCO%each index showed a trend of decrease(r=0.221,0.194,0.179,0.185,0.196,P<0.05),RV/TLC target is on the rise(r=0.262,P<0.01).No relationship of the BMI was found with age,And BMI and pulmonary function were not correlated with working age too.The patients were divided into three groups according to BMI:Low BMI group(BMI18.4kg/m^(2)or less)in 5cases,Normal BMI group(BMI18.5~24.99kg/m^(2))in 111cases,High BMI group(BMI<25kg/m^(2))in 51cases.With the increase of BMI groups,pulmonary function FEV_(1),PEF,DLCO%each indicators has increasing trend(r=0.186,0.181,0.264,P<0.05),RV/TLC indicators falling(r=0.178,P<0.05).FEV_(1),PEF,MVV and DLCO in the low BMI group were lower than those in the normal BMI group and the high BMI group,while RV/TLC was higher than those in the other two groups(P<0.01).Conclusions BMI is significantly correlated with pulmonary function,and high BMI of the pneumoconiosis patients have better pulmonary function.BMI can be used to assess the severity of pneumoconiosis.
