摘 要:目的:探讨T2-mapping成像在早期踝关节距骨骨软骨损伤(OLT)中的诊断价值。方法:对2016-2019年来我院就诊的48例怀疑OLT病例,采用西门子Verio 3.0T磁共振进行检查,设立研究组(T2-mapping成像加常规序列)和对照组(常规序列),研究组测量丁2-mapping伪彩图的软骨T2值,进行统计学分析,比较正常与损伤的差异性。结果:①研究组诊断OLT分期:0期(正常)、1〜3期(轻度损伤)、4〜5期(重度损伤)的平均丁2值分别为(22.41±3.04)ms、(34.03±4.92)ms、(41.11±5.61)ms,损伤与正常关节软骨表面T2值有差异且具统计学意义(P<0.05);②研究组正常12例,轻度损伤28例,重度损伤8例;对照组正常18例,轻度损伤22例,重度损伤8例;研究组较对照组发现轻度损伤OLT个数增多,重度损伤无差异。结论:T2-mapping成像能发现没有发生明显形态学改变的关节软骨损伤,对早期OLT的诊断具有很高的临床应用价值。Purpose:To explore the diagnostic value of MR T2-mapping imaging in early osteochondral lesions of the talus(OLT).Methods:Forty-eight cases of suspected OLT in our hospital from 2016 to 2019 were examined by Siemens Verio 3.0T magnetic resonance scanner.The study group(T2-mapping imaging plus routine sequence)and the control group(routine sequence)were set up.The cartilage T2 value was measured,and the statistical analysis was made to compare the difference between normal and injury.Results:① The T2 values of different OLT stages in the study group were:(22.41±3.04)ms for the stageO(normal group),34.03±4.92 ms for the mild stage l-3(injury group)and(41.11±5.61)ms for the stage 4-5(severe injury group),respectively,and there were significant differences of T2 values between the injured and normal articular cartilage surface(P<0.05).② Twelve cases in the study group were normal,28 cases with mild injury and 8 cases with severe injury;and 18 cases in the control group were normal,22 cases with mild injury and 8 cases with severe injury.Compared with the control group,the number of OLT with mild injury was higher in the study group,but there was no difference in the number of OLT with severe injury.Conclusions:T2-mapping imaging can be used to detect articular cartilage injury without obvious morphological changes,which has high clinical value in the early diagnosis of OLT.
