玉屏风散合知柏地黄汤治疗成人肾病综合征临床观察

摘　　要：目的探讨玉屏风散合知柏地黄汤加减治疗成人肾病综合征的临床效果及安全性。方法选取2017年1月—2019年1月收治的74例肾病综合征患者,按随机数字表法分为对照组(37例,激素治疗)和观察组(37例,玉屏风散合知柏地黄汤加减治疗),分别比较2组患者治疗效果、复发情况、肾功能、24 h尿蛋白定量、肝功能、不良反应发生情况。结果观察组的治疗效果优于对照组;观察组的复发率2.70%低于对照组的24.32%;2组治疗后尿素氮(BUN)、血肌酐(Scr)、24 h尿蛋白定量(24 h UTP)均低于治疗前,且观察组比对照组明显更低,以上差异均有统计学意义(P<0.05);观察组和对照组治疗后血浆白蛋白(ALB)、丙氨酸氨基转移酶(ALT)、谷草转氨酶(AST)与治疗前相比均变化不大,差异无统计学意义(P>0.05);观察组的不良反应发生率8.11%低于对照组的29.73%,差异有统计学意义(P<0.05)。结论采用玉屏风散合知柏地黄汤加减治疗成人肾病综合征的效果显著,患者疾病不易复发,不良反应发生率低,安全性较高,临床应用价值高。Objective To explore the clinical effect and safety of Yupingfeng powder and Zhibai Dihuang decoction in the treatment of adult nephrotic syndrome.Methods A total of 74 patients with nephrotic syndrome admitted in our hospital from January 2017 to January 2019 were selected as the research objects.They were divided into the control group(n=37,hormone therapy)and the observation group(n=37,Yupingfeng powder and Zhibai Dihuang decoction).The treatment effect,recurrence,renal function,24-hour urine protein quantification,liver function,and adverse reactions in the two groups was compared.Results The therapeutic effect of the observation group was better than that of the control group.The recurrence rate of the observation group was 2.70%,which was lower than that of the control group(24.32%).BUN(urea nitrogen),Scr(blood creatinine),24 UTP(24-hour urinary protein quantification)were lower than before treatment,and the observation group was significantly lower than the control group;the above differences were statistically significant(P<0.05).ALB(plasma white),ALT(alanine aminotransferase),AST(aspartate aminotransferase)were not significantly changed compared with before treatment,and the difference was not statistically significant(P>0.05).The incidence of adverse reactions in the observation group was 8.11%,which was lower than that in the control group(29.73%),and the difference was statistically significant(P<0.05).Conclusion The effect of using Yupingfeng powder and Zhibai Dihuang decoction in treating nephrotic syndrome in adults is significant.The disease is not easy to relapse,the incidence of adverse reactions is low,the safety is high,and the clinical application value is high.
