摘 要:目的观察缬沙坦不同使用时限对2型糖尿病肾病患者的尿微量白蛋白排泄率和肾血流动力学的影响,探讨缬沙坦对于2型糖尿病肾病患者的治疗时间方案。方法选取高州市人民医院2018年7月至2019年6月收治的2型糖尿病肾病患者100例,按随机数字表法分为对照组和观察组,每组各50例。两组均服用缬沙坦进行治疗,对照组坚持治疗12周,观察组坚持治疗24周。观察两组患者治疗前后的尿微量白蛋白(m-ALB)、尿微量白蛋白排泄量(UAER)、血肌酐(Scr)、尿素氮(BUN)、肾全血流量(RBF)和肾阻力指数(RI)变化,比较两组不良反应发生情况。结果两组患者治疗后的m-ALB、UAER、Scr、BUN、RBF和RI与治疗前比较,均明显改善(P<0.05)。观察组治疗后的m-ALB、UAER、Scr、BUN、RBF和RI值均显著优于对照组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论2型糖尿病肾病患者持续服用缬沙坦治疗24周,可有效减少尿微量白蛋白排泄量,改善患者的肾功能和肾血流动力学相关指标,值得临床推广应用。Objective To observe the impacts of different medication duration of valsartan on urinary microalbuminuria excretion rate and renal hemodynamics in patients with type 2 diabetic nephropathy,and to investigate the therapeutic time scheme of valsartan for patients with type 2 diabetic nephropathy.Methods A total of 100 patients with type 2 diabetic nephropathy admitted to the People’s Hospital of Gaozhou from July 2018 to June 2019 were divided into the control group(n=50)and the observation group(n=50)according to the random number table method.Both groups were treated with valsartan.The control group was treated persistently for 12 weeks,while the observation group was treated persistently for 24 weeks.The changes of urinary microalbumin(m-ALB),urinary microalbumin excretion rate(UAER),serum creatinine(Scr),blood urea nitrogen(BUN),renal total blood flow(RBF)and renal resistance index(RI)were observed before and after treatment,adverse reactions were compared between the two groups.Results The values of m-ALB,UAER,Scr,BUN,RBF and RI in both groups of patients were significantly improved after treatment compared with those before treatment(P<0.05).The values of m-ALB,UAER,Scr,BUN,RBF and RI in the observation group after treatment were significantly superior to those in the control group(P<0.05).There was no statistically significant difference in the overall incidence of adverse reactions between the two groups(P>0.05).Conclusion Patients with type 2 diabetic nephropathy treated persistently with valsartan for 24 weeks can effectively reduce the UAER and improve the related indexes of renal function and renal hemodynamics,which is worthy of clinical promotion and application.
