摘　　要：目的探讨活血补肾汤联合自体髂骨打压植骨在早期股骨头坏死保髋治疗中的应用价值。方法回顾性分析2017年3月至2019年3月于山东省鱼台县人民医院骨伤科住院治疗的76例早期股骨头坏死患者资料,根据治疗方案分为观察组(n=38)和对照组(n=38)。对照组采用髓芯减压+自体髂骨打压植骨治疗,观察组在对照组基础上加用活血补肾汤,总疗程3个月。比较两组各项观察指标。结果两组治疗后各项中医症候积分、血液流变学指标、炎症因子水平低于治疗前,股骨头不同区域骨密度高于治疗前,观察组变化更明显(P<0.05);观察组治疗总有效率高于对照组(P<0.05)。结论活血补肾汤联合自体髂骨打压植骨在早期股骨头坏死保髋治疗中的近期效果良好,可有效缓解关节疼痛、改善血液流变学、减轻炎症反应、增加股骨头骨密度。Objective To investigate the application value of Huoxue Bushen Decoction combined with autogenous iliac bone graft in hip-preserving treatment of early femoral head necrosis.Methods The data of 76 patients with early femoral head necrosis admitted to the department of orthopedics and traumatology of Yutai County People's Hospital,Shandong from March 2017 to March 2019 were analyzed retrospectively,and they were divided into the observation group(n=38)and the control group(n=38)according to the treatment plan.The control group was treated with core decompression combined with autogenous iliac bone graft,while the observation group was treated with Huoxue Bushen Decoction on the basis of the control group,with a total course of treatment for 3 months.Meanwhile,the observation indexes between the two groups were compared.Results After treatment,the scores of TCM symptoms,hemorheological indexes and inflammatory factors in the two groups were lower than those before treatment,and the bone mineral density in different areas of femoral head was higher than that before treatment.These differences were more obvious in the observation group(P<0.05).The total efficacy rate of the observation group was higher than that of the control group(P<0.05).Conclusion The short-term efficacy of Huoxue Bushen Decoction combined with autogenous iliac bone graft in the treatment of early femoral head necrosis with hip-preserving is good,which can effectively relieve joint pain,improve hemorheology,reduce inflammatory reaction and increase bone density of femoral head.
