摘 要:目的比较几种不同囊袋切口的胸壁完全性植入式输液港的手术效果。方法回顾性分析2018年1月至2019年7月期间158例于广州市第一人民医院接受输液港植入术的患者资料。根据术式不同分为3组,隧道针-横切口组(n=47)、穿刺点-纵切口组(n=60)、穿刺点-横切口组(n=51),对3种术式的手术时间、术后并发症进行对比。结果3种术式中平均手术时间,隧道针-横切口组最长为(31.36±1.10)min,其次为穿刺点-横切口组(28.65±1.07)min,穿刺点-纵切口组最短为(26.70±0.93)min,差异具有统计学意义(P<0.05)。并发症方面,隧道针-横切口组5例(10.4%),穿刺点-纵切口组10例(16.7%),穿刺点-横切口组7例(13.7%),差异无统计学意义(P>0.05)。结论穿刺点-纵切口、穿刺点-横切口两种术式操作更为简便,是锁骨下静脉胸壁完全性植入式输液港的理想术式。Objective To investigate the effect of three different subcutaneous capsular bags in totally implantable venous access port.Methods From January 2018 to July 2019,three different types of subcutaneous capsular bags were adopted in 158 cases of totally implantable venous access port implantation.The patients were divided into three groups according to the different operation including tunnel needle-transverse incision group(n=47),puncture point-transverse incision group(n=51)and puncture point-longitudinal incision group(n=60).The operation time and complications were compared among the three groups.Results The average time of the three surgical methods was(31.36±1.10)min in the tunnel needle-transverse incision group,(28.65±1.07)min in the puncture point-transverse incision group and(26.70±0.93)min in the puncture point-longitudinal incision group(P<0.05).In terms of complications,there were 5 cases(10.4%)in the tunnel needle-transverse incision group,10 cases(16.7%)in the puncture point-longitudinal incision group,and 7 cases(13.7%)in the puncture point-transverse incision group.The difference of incidences was not significant(P>0.05).Conclusions The methods of puncture point-longitudinal and puncture point-transverse incision take the less operation time without increasing complications which may be the preferable procedures for totally implantable venous access port implantation.
