摘　　要：目的探讨腹腔镜手术对急性化脓性阑尾炎患者免疫功能及血清白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平的影响。方法选取2018年2月~2019年10月辽宁省健康产业集团抚矿总医院收治的89例急性化脓性阑尾炎患者,按随机数字表法分为对照组(n=44)和观察组(n=45)。对照组进行常规开腹手术,观察组进行腹腔镜术。比较两组的临床指标和术前、术后72 h免疫功能、血清IL-8、TNF-α的水平。结果术后72 h观察组的切口长度、排气时间短于对照组,且术中出血量少于对照组;术后72 h两组的血清免疫球蛋白M(IgM)、免疫球蛋白G(IgG)水平低于术前,观察组的术后72 h血清IgM、IgG水平高于对照组;且术后72 h两组的血清IL-8、TNF-α水平低于术前,观察组术后72 h血清IL-8、TNF-α水平低于对照组,差异均有统计学意义(P<0.05)。结论腹腔镜术治疗急性化脓性阑尾炎作用显著,可改善患者免疫功能,同时降低血清IL-8、TNF-α水平,抑制机体炎症反应,促进患者恢复。Objective To explore the effect of laparoscopic surgery on immune function and serum levels of interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)in patients with acute suppurative appendicitis.Methods A total of 89 patients with acute suppurative appendicitis treated in Liaoning Health Industry Group Fukuang General Hospital from February 2018 to October 2019 were selected,and divided into the control group(n=44)and the observation group(n=45)by the random number table method.The control group underwent routine laparotomy,and the observation group underwent laparoscopic surgery.The clinical indicators were compared between the two groups,and the immune function,serum IL-8,and TNF-αlevels before and 72 h after operation were compared between the two groups.Results The incision length and exhaust time of the observation group were shorter than those of the control group,and the amount of bleeding was less than that of the control group at 72 h after operation;at 72 h after operation,the serum levels of immunoglobulin M(IgM)and immunoglobulin G(IgG)in the two groups were lower than those before operation,and the serum IgM and IgG levels in the observation group were higher than those in the control group at 72 h after operation;at 72 h after operation,the levels of serum IL-8 and TNF-αin the two groups were lower than those before operation,and the levels of serum IL-8 and TNF-αin the observation group were lower than those in the control group at 72 h after operation,the differences were statistically significant(P<0.05).Conclusion Laparoscopic surgery has a significant effect on the treatment of acute suppurative appendicitis,which can improve the patient's immune function,reduce the serum IL-8 and TNF-αlevels,inhibit the body's inflammatory response,and promote the patient's recovery.
