摘 要:目的探究慢性阻塞性肺疾病(COPD)患者不规则趋化因子(FKN)、白三烯B4(LTB4)及呼出气一氧化氮(FeNO)水平变化及其临床意义。方法选择2019年4月至2020年4月在我院就诊的120例COPD患者,另选取同期健康体检者60例作为对照组,按病情严重程度将120例患者分为COPD急性加重期患者(n=64)与稳定期患者(n=56)。比较三组第1秒用力呼气容积在预计值的百分比(FEV1%pred)、一秒率(FEV1/FVC)、FKN、LTB4及FeNO;采用Pearson法分析FKN、LTB4及FeNO指标水平与FEV1%pred、FEV1/FVC的相关性。结果COPD组患者FEV1%pred、FEV1/FVC水平明显低于对照组,FKN、LTB4、FeNO水平明显高于对照组(P<0.05);COPD急性加重期组患者FEV1%pred、FEV1/FVC水平明显低于稳定组,FKN、LTB4、FeNO水平明显高于稳定组(P<0.05);COPD患者FKN、LTB4水平与FEV1%pred及FEV1/FVC均呈负相关(P<0.05),FeNO水平与FEV1%pred及FEV1/FVC无相关性(P>0.05)。结论FKN、LTB4、FeNO水平与COPD病情严重程度密切相关,随着患者病情的加重呈升高趋势,可为临床准确评估COPD患者病情程度、制定治疗方案提供可靠信息。Objective To explore the changes of irregular chemokine(FKN),leukotriene B4(LTB4)and exhaled nitric oxide(FENO)levels in patients with chronic obstructive pulmonary disease(COPD)and their clinical significances.Methods A total of 120 COPD patients who were treated in our hospital from Apr.2019 to Apr.2020 were selected for the study,and another 60 healthy physical examination patients during the same period were selected as the control group.According to the severity of the disease,120 COPD patients were divided into the patients with acute exacerbation of COPD group(n=64)and the stable patient group(n=56).The percentage of forced expiratory volume(FEV1%pred),one second rate(FEV1/FVC),FKN,LTB4 and FeNO in the three groups were then compared.Pearson method was used to analyze the correlation between FKN,LTB4 and FeNO and FEV1%pred and FEV1/FVC.Results The levels of FEV1%pred and FEV1/FVC in the COPD group were significantly lower than those in control group,while FKN,LTB4 and FeNO levels were significantly higher than those in control group(P<0.05).FEV1%pred and FEV1/FVC levels in the COPD acute exacerbation group were significantly lower than those in the stable group,while FKN,LTB4 and FeNO levels were significantly higher than those in the stable group(P<0.05).The levels of FKN and LTB4 were negatively correlated with FEV1%PRED and FEV1/FVC in COPD patients(P<0.05),while FeNO level was not correlated with FEV1%PRED and FEV1/FVC(P>0.05).Conclusion The levels of FKN,LTB4,and FeNO are closely related to the severity of COPD,and tend to increase as the patient′s condition worsens,which can provide a reliable information for clinically accurate assessment of COPD patients′condition and the development of treatment plans.
