可吸收螺钉治疗踝关节骨折的临床研究

摘　　要：目的:比较可吸收螺钉与金属螺钉治疗踝关节骨折的疗效。方法:选取本院2017年7月~2019年7月踝关节骨折患者72例,根据入院顺序随机分为观察组和对照组各36例。观察组采用可吸收螺钉内固定治疗,对照组采用金属螺钉内固定治疗,术后指导患者行踝关节功能锻炼。观察两组治疗后的疗效和并发症的发生率。结果:随访时间12~24个月,平均18个月。两组患者切口均Ⅰ期愈合,未出现异物反应病例,随访期间两组患者踝关节骨折均骨性愈合,无复位丢失。对照组出现2例金属螺钉在皮下对周围软组织产生激惹反应从而出现疼痛,二次手术取出金属螺钉时有2例术中出现螺钉滑丝。两组疗效比较差异无统计学意义(P>0.05)。结论:可吸收螺钉在治疗踝关节骨折方面提供了充分的生物力学稳定性,具有生物组织相容性良好、避免二次手术取内固定物的优点,是一种理想的内固定材料,值得临床进行推广应用。Objective:To compare the clinical efficacy of absorbable screws and metal screws in the treatment of ankle fractures.Methods:A total of 72 patients with ankle fractures in our hospital from July 2017 to July 2019 were selected and randomly divided into observation group and control group according to the admission sequence,with 36 patients in each group.The observation group was treated with absorbable screw internal fixation,while the control group was treated with metal screw internal fixation.The patients were guided to conduct ankle functional exercise after operation.The curative effect and complication rate of the two groups were observed.Results:The follow-up time was 12~24 months,with an average of 18 months.Two groups of patients are I healing of incision,no foreign body reaction cases occurred,two groups of patients with ankle fractures were osseous healing during the follow-up period,no reset lost.In the control group,2 cases of metal screws had pain due to the subcutaneous irritating reaction to the surrounding soft tissue,and 2 cases had screw sliders during the second operation when the metal screws were removed.There was no statistically significant difference in efficacy between the two groups(P>0.05).Conclusion:The absorbable screw provides sufficient biomechanical stability in the treatment of ankle fractures.It has the advantages of good histocompatibility and avoiding secondary operation.It is an ideal internal fixation material,which is worth applying in clinic.
