摘 要:目的探讨2015-2018年杭州地区手足口病(HFMD)的流行病学特征、病原学特点以及HFDM发病率与气象因素的关系。方法回顾性统计2015-2018年杭州地区HFMD的临床病例进行流行病学特点分析;利用实时荧光定量聚合酶链反应检测病毒核酸;多元线性回归对HFMD阳性率与气象因素进行相关性分析。结果2015-2018年共临床诊断HFMD通用型阳性病例2652例,其中柯萨奇病毒A16型(CA16)8例,阳性率为0.30%,未检出肠道病毒71型(EV71)。HFMD发病时间聚集以5~7月份为高峰,平均年龄(2.45±1.66)岁,男女比例为1.6∶1。多元线性回归分析显示,肠道病毒阳性率与平均气温、相对湿度、降雨均呈正相关(P<0.05);与日照时数无明显相关性(P=0.203)。结论杭州地区2015-2018年HFMD仍以1~3岁散居男童为主,病原学以肠道病毒通用型为主;HFMD发病率与平均气温、相对湿度、降雨量密切相关,与日照时数无明显相关性。Objective To explore the epidemiological and etiological characteristics of hand-foot-mouth disease(HFMD)in Hangzhou from 2015 to 2018,and the relationship between the incidence of HFMD and meteorological factors.Methods The epidemiological features of HFMD patients in Hangzhou area in 2015-2018 were analyzed.The real-time fluorescence quantitative polymerase chain reaction was performed to detect the levels of virus nucleic-acid.The correlation between the positive rate of HFMD and meteorological factors was analyzed by multiple linear regression.Results From 2015 to 2018,2652 cases of HFMD were clinically diagnosed as universal positive cases,including 8 cases of Coxsackievirus A16(CA16),the positive rate was 0.30%,and no enterovirus 71(EV71)was detected.The peak time of HFMD cases was from May to July,with an average age of(2.45±1.66)years and 1.6∶1 for men and women.Multiple linear regression analysis showed that the positive rate of enterovirus was positively correlated with mean temperature,relative humidity and rainfall(P<0.05),but not with sunshine hours(P=0.203).Conclusion The prevalence of HFMD in Hangzhou is still mainly among scattered boys aged 1-3 years,and enterovirus is the main pathogen.The incidence of HFMD is closely related to average temperature,relative humidity and rainfall,but not to sunshine hours.
