摘  要:目的探索有效抗病毒治疗(ART)的男性艾滋病病毒(HIV)感染者血液和精液细胞中HIV辅助受体使用情况。方法对成功ART时间≥6个月病人采集抗病毒治疗前后的外周血和精液,离心提取外周血中单个核细胞和精液细胞,提取标本中全部HIV脱氧核糖核酸(DNA)。对HIV包膜的C2-V5区进行克隆和测序,使用在线工具Geno2phen以及WebPSMM共同预测CXCR4或CCR5辅助受体,env区序列的亚型鉴定使用在线分析工具Los Alamos HIV数据库。用SPSS 23.0进行统计分析。结果78例入选HIV感染男性病人ART前CRF01_AE亚型、B和CRF07_BC亚型使用CXCR4的比率分别为47.22%、3.08%和2.00%,精液中三个亚型的辅助受体使用分别为44.12%、3.85%和3.33%,不同亚型血液、精液中辅助受体的使用比较均有统计学意义(P<0.001),但是同一个亚型血液、精液中辅助受体使用的比较差异均无统计学意义(P>0.05)。ART后,所有亚型血液和精液中均测不到辅助受体CXCR4的使用,100%为CCR5辅助受体。结论成功ART前后精液和血液中HIV辅助受体使用均无差异,但是不同亚型之间使用辅助受体差别较大,ART后均转为CCR5辅助受体。Objective To explore the use of latent HIV receptors in blood and semen cells of male patients with successful antiretroviral therapy(ART).Methods Peripheral blood and semen of patients with successful ART≥6 months were collected before and after ART,PBMC and semen cells were extracted by centrifugation,and all HIV DNA was extracted from the samples.The C2-V5 region of HIV envelope was cloned and sequenced.Geno2 phen and webpsmm were used to predict the CXCR4 or CCR5 receptor usage.Los Alamos HIV database was used for subtype identification of Env region sequence.Spss23.0 was used for statistical analysis.Results A total of 78 HIV male patients were enrolled in this study.The proportion of CXCR4 used in CRF01_AE,B and BC subtype were 47.22%,3.08%and 2.00%respectively(P<0.001);and the co-receptor use of three subtypes in semen was 44.12%,3.85%and 3.33%respectively(P<0.001).Differences in the use of CXCR4 in blood and semen of the same subtype were all P>0.05.After ART,use of CXCR4 was not detected in blood and semen of all subtypes,and 100%of them were CCR5.Conclusion There is no significant difference in the use of HIV co-receptor in semen and blood before and after ART,but there is a big difference in the use of co-receptor between different subtypes.After ART,all of them turn to CCR5 co-receptor usage.
