摘　　要：目的探讨艾滋病相关肺部卡波西肉瘤影像表现特征,提高对该疾病的诊断及鉴别诊断能力。方法回顾性分析本院经病理证实的15例艾滋病合并卡波西肉瘤病人的临床资料及影像学表现。结果15例艾滋病合并卡波西肉瘤病人中,14例为男性,均为男男性行为感染;11例结节型卡波西肉瘤,表现为沿支气管血管束分布的火焰样结节影,其中4例病人病灶周围可见磨玻璃密度;2例支气管血管束增粗型,表现为支气管管壁增厚,呈"袖套"样;2例肺炎渗出型,表现以斑片渗出为主,同时伴多发火焰样结节、小叶间隔增厚及多发斑片状磨玻璃密度影;11例合并纵隔及双侧腋窝淋巴结增大,5例合并胸腔积液,3例合并心包积液。结论艾滋病合并肺部卡波西肉瘤的影像学表现具有特征性,表现为沿支气管血管束分布的"火焰样"结节及斑片影。Objective To analyze the imaging features of pulmonary Kaposi’s sarcoma and improve the ability of diagnosis and differential diagnosis of the disease.Methods Clinical and imaging features of 15 cases pathologically confirmed by Kaposi’s sarcoma with AIDS were analyzed retrospectively.Results There were 14 male patients,and all of them were infected by sex with men.11 cases of nodular Kaposi’s sarcoma showed flame like nodular distributed along the bronchovascular bundles,ground glass opacity was seen around the lessions in 4 cases;2 cases of thickened bronchovascular bundle,which showed the sleeve like thickening of bronchial wall;2 cases of exudative pneumonia type showing flame like patchy infiltration,at the same time there were multiple flame like nodulaes,interlobular septal thickening and multiple patchy ground glass opacity.Among them,11 cases had mediastinal and bilateral axillary lymph nodes enlargement;5 cases were complicated with pleural effusion and 3 cases complicated with pericardial effusion.Conclusion The imaging findings of pulmonary Kaposi’s sarcoma are characteristic,mainly manifested as"flame like"nodules and patchy shadows distributed along the bronchovascular bundles.
