摘 要:目的研究体外循环对孕山羊雌孕激素及胎盘功能的影响。方法取健康孕山羊10头,麻醉后用膜式氧合器和搏动泵建立胎羊体外循环并持续30 min,期间分别于建立体外循环前及建立体外循环后的15 min、30 min时取部分胎盘小叶、采动静脉血,胎盘小叶用于胎盘功能检测,动静脉血液用于血气分析及雌孕激素水平的检测。结果体外循环过程中孕山羊与胎羊心跳有力,血压正常,氧分压及二氧化碳分压正常稳定。雌二醇激素水平逐渐升高(T1:93.3±15.6 pg/ml;T5:128.7±14.4lpg/ml),孕酮水平逐渐下降(T1:19.6±2.2 ng/ml;T5:8.4±1.1 ng/ml),胎羊pH值(T1:7.40±0.08;T5:7.11±0.15)、氧分压逐渐下降(T1:25.3±1.8 mmHg;T5:18.1±2.1 mmHg),二氧化碳分压逐渐升高(T1:47.1±1.4 mmHg;T5:61.9±10.8 mmHg)。进一步的胎盘功能学检测显示,随着体外循环的进行,胎盘组织中内皮型一氧化氮合酶(endothelial nitric oxide synthasee,NOS)的表达及一氧化氮(nitric oxide,NO)含量出现先高后低的变化规律。结论体外循环对雌孕激素和胎盘功能的影响可能不利于胎羊的存活。Objective To study the effect of cardiopulmonary bypass on estrogen,progesterone and placental fiinction in pregnant goats.Methods Ten healthy pregnant goats were selected.After anesthesia,fetal sheep cardiopulmonary bypass were established with membrane oxygenator and roller pump for 30 minutes.During the period,part of placenta lobules and the blood were taken before and at 15 minutes and 30 minutes after the establishment of cardiopulmonary bypass.Placenta lobules were used for placental function test,arterial and venous blood were used for estrogen and progesterone testing.Results During cardiopulmonary bypass,the heartbeat of pregnant and fetal goats were strong,with normal blood pressure,oxygen partial pressure and carbon dioxide partial pressure.The level of estradiol increased[T1:(93.3±15.6)pg/ml;T5:(128.7±14.41)pg/ml j,but the level of progesterone decreased[Tl:(19.6±2.2)ng/ml;T5:(8.4±1.1)ng/ml ];the pH value(T1:7.40±0.08;T5:7.11±0.15)and PO:[Tl:(25.3±1.8)mm Hg;T5:(18.1±2.1)mm Hg[of fetal sheep decreased,but PCOincreased T1:(47.1±1.4)mm Hg][T5:(61.9±10.8)mm Hg ].Placental function showed that the expression of endothelial nitric oxide synthetase(eNOS)and the content of nitric oxide(NO)in the placental decreased tissue with cardiopulmonary bypass.Conclusion The effect of external circulation on estrogen,progesterone and placental function may be harmful.
