摘  要:目的探讨持续空气阻隔法对减少静脉输液冲管液量的效果。方法选取临床上常用的容易发生配伍反应并能采用光吸收酶标仪测定药物吸光度的6种药物作为试验对象,分别为注射用更昔洛韦、注射用泮托拉唑、依达拉奉注射液、盐酸氨溴索注射液、呋塞米注射液、注射用头孢曲松钠,均按照临床常用浓度配置药液,分别采用持续空气阻隔法与传统冲管法进行体外模拟实验,利用光吸收酶标仪测定不同冲管液量组标本的吸光度,采用与溶剂一致的0.9%氯化钠注射液作为空白对照组,当不同冲管液量组与空白对照组标本吸光度比较差异有统计学意义(P<0.05)时,判定输液管内仍有残留药液;当不同冲管液量组与空白对照组标本吸光度比较差异无统计学意义(P>0.05)时,判定输液管内残留药液已被冲洗干净,该标本对应的冲管液量为最小有效冲管液量,即为该药物合适的冲管液量。结果6种药物采用传统冲管法,不同冲管液量组分别为25.5 ml、25.5 ml、25.5 ml、31.5 ml、34.5 ml、43.5 ml时,其吸光度和空白对照组比较差异无统计学意义(P>0.05),此时输液管内残留药液被完全冲洗干净。采用持续空气阻隔法,冲管液量分别为20 ml、20 ml、20 ml、20 ml、23 ml、29 ml时,其吸光度和空白对照组比较差异无统计学意义(P>0.05),此时输液管内残留药液被完全冲洗干净。结论持续空气阻隔法是一种安全有效的冲管方法,较传统冲管法能够显著减少静脉输液冲管液量,值得在临床推广。Objective To explore the use of continuous air barrier approach in reducing the flush volume for intravenous infusion tubing.Methods The study drugs included injectable solutions of six clinically common drugs that are prone to compatibility reactions and can be measured with a light absorption microplate reader:ganciclovir,pantoprazole,edaravone,ambroxol hydrochloride,furosemide,and ceftriaxone sodium.The drugs were prepared to usual concentrations commonly used in clinical practice.In vitro simulation of infusion tube flushing was performed by continuous air barrier approach and conventional flushing approach.A light absorption microplate reader was used to measure the absorbance of the specimen with different flush volumes.Matched volumes of 0.9%sodium chloride solution were used as blank controls.Drug residue in the infusion tubing was determined when a statistically significant difference(P<0.05)in the absorbance was noted between a given specimen and the corresponding blank control,otherwise complete washout of drug from the infusion tubing was considered.The minimal effective flush volume needed for complete washout of drugs in a specimen was regarded as suitable flush volumes.Results Via conventional flushing approach,when the flush volume for these six drugs was 25.5 ml,25.5 ml,25.5 ml,31.5 ml,34.5 ml,and 43.5 ml,respectively,there was no significant difference in absorbance compared with the blank control(P>0.05),suggesting complete washout of the drug residue from the infusion tubing.Via continuous air barrier approach,when the flush volume for these six drugs was 20 ml,20 ml,20 ml,20 ml,23 ml,29 ml,there was no significant difference in absorbance compared with the blank control(P>0.05),suggesting complete washout of the drug residue from the infusion tubing.Conclusion Continuous air barrier approach is a safe and effective method for tube-flushing,with significantly less flush volume for intravenous infusion tubing compared with conventional flushing approach.This justifies its widespread use in clin
