胎儿心脏外翻畸形产前超声诊断及临床价值

摘　　要：目的:探讨胎儿心脏外翻畸形产前超声诊断及临床价值。方法:回顾性分析我院2015年1月-2020年2月产前超声检查发现3例胎儿心脏外翻畸形声像图及临床资料,并结合文献复习胎儿心脏外翻的超声诊断及预后。结果:2例晚孕期胎儿表现为心脏完全裸露胸腔外,其中1例合并右心发育不良、肺动脉瓣狭窄及胎儿唇裂畸形;1例合并单心室、近似单心房、单组房室瓣、大动脉异位及肺静脉异位引流;1例12周早孕期胎儿前胸腹裂,心脏通过发育不良的膈肌下移至腹腔,并与肝脏一起外翻畸形,胎儿皮肤水肿综合征,双足内翻畸形,染色体18-三体综合征。3例均终止妊娠。结论:产前超声检查可以早期发现胎儿心脏外翻畸形以及胎儿其他组织器官的畸形,为临床产前诊断及治疗提供可靠的影像学依据。Objective:To explore the prenatal ultrasound diagnosis and clinical value offetal cardiac valgus deformity.Methods:Retrospective analysis of our hospital from January 2015 to February 2020,prenatal ultrasound examination found 3 cases of fetal cardiac valgus deformity sonogram and clinical data,combined with literature review fetal cardiac valgus ultrasound diagnosis and prognosis.Results:Two fetuses in the third trimester showed complete exposure of the heart outside the thoracic cavity.One of them had right heart dysplasia,pulmonary valve stenosis,and fetal cleft lip malformation;one had a single ventricle,similar to a single atrium,a single group of atrioventricular valves,and anomalous large arteries.Pulmonary vein ectopic drainage;1 case of fetus with prothoracic fissure in the first trimester of 12 weeks,the heart moved down to the abdominal cavity through the dysplastic diaphragm,and together with the liver valgus deformity,fetal skin edema syndrome,bipedal varus deformity,chromosome 18-Trisomy.All 3 cases terminated their pregnancy.Conclusion:Prenatal ultrasound can detect fetal heart ectropion and other malformations of fetal tissues and organs early,and provide a reliable imaging basis for clinical prenatal diagnosis and treatment.
