右美托咪定复合依托咪酯在老年无痛胃肠镜中的应用效果及对应激反应的影响

摘　　要：目的探讨右美托咪定复合依托咪酯在老年无痛胃肠镜中的应用效果及对应激反应的影响。方法选择2018年5月~2019年5月葫芦岛市中心医院行胃肠镜检查的92例老年患者作为研究对象。按照随机数字表法分为对照组和观察组,每组各46例。对照组给予依托咪酯麻醉,观察组给予右美托咪定复合依托咪酯麻醉。比较两组检查前后的皮质醇(Cor)、醛固酮(ALD)、血管紧张素Ⅱ(AT-Ⅱ)水平。比较两组的自主呼吸恢复时间、苏醒时间、拔管时间、检查期间不良反应总发生率、检查后1 h的Ramsay镇静评分及疼痛视觉模拟量表(VAS)评分。结果检查后5 min和检查后1 h,观察组应激指标水平均低于对照组,差异有统计学意义(P<0.05);检查后1 h,观察组Ramsay镇静评分高于对照组,VAS评分低于对照组,差异有统计学意义(P<0.05);检查期间,观察组不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论右美托咪定复合依托咪酯老年无痛胃肠镜检查中的应用,可发挥较好的镇静、镇痛作用,并减轻患者应激反应,降低不良反应发生率。Objective To explore the application effect of Dexmedetomidine combined with Etomidate in painless gastrointestinal endoscopy and its influences on stress response in the elderly.Methods A total of 92 elderly patients undergoing gastrointestinal endoscopy in Huludao Central Hospital from May 2018 to May 2019 were selected as the research subjects.The control group was given Etomidate,while observation group was given Dexmedetomidine combined with Etomidate for anesthesia.The levels of cortisol(Cor),aldosterone(ALD),angiotensin Ⅱ(AT-Ⅱ)before examination,after 5 minutes of examination and at 1 hour after examination,recovery time of spontaneous breathing,recovery time and extubation time after withdrawal,scores of Ramsay sedation and visual analogue scale(VAS)at 1 hour after examination,and the occurrence of adverse reactions during examination were compared between the two groups.Results The levels of stress indicators in the observation group were lower than those in the control group at 5 min and 1 hour after examination,and the differences were statistically significant(P<0.05).At 1 hour after the examination,Ramsay sedation score in the observation group was higher than that in the control group,VAS score was lower than that in the control group,the differences were statistically significant(P<0.05).During the examination,the total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of Dexmedetomidine combined with Etomidate in elderly painless gastroenteroscopy can play a better sedative and analgesic effect,and alleviate the stress response of patients and reduce the incidence of adverse reactions.
