扶正化浊汤治疗老年耐药菌肺炎气虚痰浊阻肺证患者的临床效果

摘　　要：目的探讨扶正化浊汤治疗老年耐药菌肺炎(气虚痰浊阻肺证)患者的临床效果。方法选取2017年10月~2019年5月大连市中医医院收治的60例老年耐药菌肺炎(气虚痰浊阻肺证)患者作为研究对象,采用随机数字表法将患者分为治疗组和对照组,每组各30例。对照组患者采用常规西药治疗,治疗组患者在常规西药治疗的基础上给予扶正化浊汤治疗。比较治疗后两组患者的耐药菌清除率、临床肺部感染评分(CPIS)、炎症指标[肿瘤坏死因子-α(TNF-α)、高迁移率族蛋白B1(HMBG-1)、白介素-10(IL-10)]。结果两组的耐药菌清除率比较,差异无统计学意义(P>0.05);治疗组CPIS低于对照组,差异有统计学意义(P<0.05);治疗组患者血清TNF-α、HMBG-1浓度水平低于对照组,差异有统计学意义(P<0.05);治疗组患者血清IL-10浓度水平高于对照组,差异有统计学意义(P<0.05)。结论在常规西药治疗基础上给予扶正化浊汤治疗老年耐药菌肺炎抗感染的效果优于单纯常规西药组,且在清除炎症介质方面有积极、有效的作用。Objective To explore the clinical effect of Fuzheng Huazhuo Decoction in the treatment of elderly patients with drug-resistant bacteria pneumonia qi deficiency phlegm turbidity obstructing lung syndrome.Methods A total of 60 elderly patients with drug-resistant pneumonia(qi deficiency phlegm turbidity obstructing lung syndrome)admitted to Dalian Traditional Chinese Medicine Hospital from October 2017 to May 2019 were selected as the research objects and divided into the treatment group and the control group according to random number table method,with 30 cases in each group.The control group was treated by routine western medicine treatment,and the treatment group was treated by Fuzheng Huazhuo Decoction on the basis of the treatment of the control group.After treatment,the antibiotic resistance bacteria clearance rate,clinical pulmonary infection score(CPIS),inflammatory indicators(tumor necrosis factor-α[TNF-α],high mobility group box 1 protein[HMBG-1],and interleukin-10[IL-10])were compared between the two groups.Results There was no statistically significant difference in the clearance rate of drug-resistant bacteria between the two groups(P>0.05).The clinical pulmonary infection score(CPIS)of the treatment group was lower than that of the control group,and the difference was statistically significant(P<0.05).The serum levels of TNF-α and HMBG-1 in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05).The serum level of IL-10 in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The anti-infective effect of the treatment of elderly drug-resistant bacteria pneumonia qi deficiency phlegm turbidity obstructing lung syndrome with Fuzheng Huazhuo Decoction on the basis of the conventional western medicine,and it has a positive and effective role in clearing inflammatory mediators.
