尘肺治疗中使用经纤支镜大容量全肺灌洗术的疗效观察

摘　　要：目的:观察经纤支镜大容量全肺灌洗术治疗尘肺疗效。方法:选取180例(2018年1月~2019年12月)尘肺患者,给予经纤支镜大容量全肺灌洗术治疗,比较临床效果。结果:治疗后咳嗽、气短;咳嗽、胸痛;咳嗽、胸闷、胸痛患者临床症状均得到改善(P<0.05),虽然咳嗽、咳痰中8例未好转,咳嗽、胸闷9例未好转,但好转率均较高(P<0.05)。患者FVC、FEV1均有提高,虽然FEV1治疗前后差异不显著(P>0.05),但FVC较治疗前有显著提高(P<0.05),肺通气功能得到改善。部分患者胸部X线片显示肺纹理变清晰,且肺部病变密集度较治疗前有所减轻;部分患者出现不良反应,给予相应处理后得到缓解和消失。总满意率93.33%。结论:将经纤支镜大容量全肺灌洗应用在尘肺患者治疗中,取得了不错的疗效。Objective:To observe the effect of large-volume whole lung lavage by bronchoscopy in the treatment of pneumoconiosis.Methods:180 patients(January 2018 to December 2019)with pneumoconiosis were selected and treated with bronchofiberoscope large-volume whole lung lavage,and the clinical effects were compared.Results:After treatment,cough,shortness of breath;cough,chest pain;clinical symptoms of patients with cough,chest tightness,and chest pain were all improved(P<0.05),although 8 cases of cough and sputum did not improve,9 cases of cough and chest tightness did not improve,but the improvement rate was higher(P<0.05).The patients'FVC and FEV1 were improved.Although the difference before and after FEV1 treatment was not significant(P>0.05),the FVC was significantly higher than before treatment(P<0.05),and the lung ventilation function was improved.The chest X-ray film of some patients showed that the lung texture became clear,and the intensity of lung lesions was less than before treatment;some patients had adverse reactions,which were relieved and disappeared after corresponding treatments.The overall satisfaction rate is 93.33%.Conclusion:The application of bronchoscopic large-volume whole lung lavage in the treatment of patients with pneumoconiosis has achieved good results.
