后凸成形术治疗骨质疏松椎骨折伴椎体裂隙征

摘　　要：[目的]评估椎体后凸成形术(kyphoplasty,KP)治疗骨质疏松椎骨折伴椎体裂隙征的临床结果。[方法]回顾分析本科采用经皮椎体后凸成形术治疗的骨质疏松椎骨折伴椎体裂隙征患者23例,分别在术前、术后第3 d及末次随访时,记录VAS、ODI评分以及并发症情况。同时在各时间点测量椎体高度和局部后凸角。[结果]23例患者均成功接受手术,其中4例患者椎间隙存在无症状性骨水泥渗漏,受累节段均集中于胸腰段。术后第3 d和末次随访时患者的VAS及ODI评分较术前显著降低,差异有统计学意义(P<0.05)。影像方面,术后第3 d局部后凸角、椎体前缘高度和椎体中缘高度均较术前显著改善,差异有统计学意义(P<0.05),末次随访与术后第3 d比较,差异无统计学意义(P>0.05)。[结论]后凸成形术治疗含气椎体裂隙征(intraverte⁃bral cleft,IVA)骨质疏松椎体骨折安全、有效。[Objective]To evaluate the clinical outcomes of percutaneous kyphoplasty(PKP)for osteoporotic vertebral fractures(OVFs)accompanied with intravertebral cleft sign.[Method]A retrospective study was conducted on 23 patients who underwent PKP for osteoprotic vertebral fracture accompanied with intravertebral cleft sign in our department from August 2015 to August 2018.The patients were evaluated by using visual analogue scale(VAS)for pain and Oswestry disability index(ODI),as well as local kyphotic angle(LKA),vertebral height anterior(Ha)and vertebral height middle(Hm)measured on radiographs before operation,at 3 days postoperatively and the latest follow up.[Results]All the patients had operations performed successfully,with asymptomatic cement leakage in 4 patients,which involved thoracolumbar segments mainly.The VAS and ODI scores significantly decreased at 3 days postoperatively and the latest follow up compared with those before operation(P<0.05).With regard to radiographic assessment,the LKA,Ha and Hm significantly improved at 3 days after operation compared with those before operation(P<0.05),and then remained unchanged to the latest follow up(P>0.05).[Conclusion]The percutaneous kyphoplasty is a safe and effective treatment for osteoporotic vertebral fractures accompanied with intravertebral cleft sign.
