宫腔镜下电切术后联合左炔诺酮对子宫内膜息肉患者激素水平、子宫内膜厚度与并发症的影响

摘　　要：目的探究宫腔镜下电切术后联合左炔诺酮对子宫内膜息肉患者激素水平、子宫内膜厚度与并发症的影响。方法选取2017年1月至2020年1月我院收治的108例子宫内膜息肉患者,采用随机数字法分为实验组(n=53)与对照组(n=55),对照组给予宫腔镜电切术治疗,术后进行常规抗感染治疗;实验组在对照组基础上加用左炔诺酮治疗。比较两组患者治疗前、治疗后1个月激素水平[卵泡生成激素(FSH)、促黄体生成素(LH)、雌激素(E2)]和子宫内膜厚度,比较两组患者治疗后并发症的发生率。结果两组患者治疗前与治疗1个月后FSH、LH、E2水平比较,差异无统计学意义(P>0.05);两组患者治疗1个月后,子宫内膜厚度均低于治疗前,且实验组子宫内膜厚度低于对照组,差异具有统计学意义(P<0.05);两组患者治疗后,实验组并发症发生率5.66%(3/53)低于对照组40.00%(22/55),差异具有统计学意义(P<0.05)。结论宫腔镜下电切术后联合左炔诺酮治疗子宫内膜息肉患者,可以有效抑制子宫内膜增生,降低并发症发生率,且不会影响患者性激素水平。Objective To explore the effect of transcervical resection combined with levonorgestrel on hormone levels,endometrial thickness and complications in patients with endometrial polyps.Methods One hundred and eight patients with endometrial polyps admitted to our hospital from January 2017 to January 2020 were enrolled and divided into the experimental group(n=53)and the control group(n=55)by random number method.The experimental group was treated with levonorgestrel on the basis of the control group;the control group was treated with transcervical resection,and conventional anti-infective treatment was performed postoperatively.The hormone levels [follicle-stimulating hormone(FSH),luteinizing hormone(LH),estrogen(E2)] and the endometrial thickness before and 1 month after treatment as well as the incidence of complications after treatment were compared.Results There was no significant difference in the levels of FSH,LH and E2 in the two groups of patients before treatment and after 1 month of treatment(P>0.05).After 1 month of treatment,the endometrial thickness of the two groups of patients was significantly lower than before treatment,and the endometrial thickness of the experimental group was significantly lower than that of the control group(P<0.05).After treatment,the incidence of complications in the experimental group was significantly lower than that in the control group [5.66%(3/53)vs.40.00%(22/55),P<0.05].Conclusion Transcervical resection combined with levonorgestrel can effectively can effectively inhibit endometrial hyperplasia,reduce the incidence of complications,and not affect the sex hormone levels in patients with endometrial polyps.
