血清Cys C、BNP表达水平与老年慢性心衰患者心室重构的相关性

摘　　要：目的探讨血清胱抑素C(Cys C)、B型脑钠肽(BNP)表达水平与老年慢性心衰患者心室重构的相关性。方法选取2019年8月-2020年9月河南科技大学第一附属医院心内科收治的70例慢性心衰老年患者作为研究组,根据NYHA分级标准将其分为Ⅱ级25例、Ⅲ级32例、Ⅳ级13例;同期选取30例于医院体检的健康人群作为对照组。测定并比较各组别血清Cys C、BNP水平及心室结构功能指标,经Pearson检验其相关性。结果研究组血清Cys C、BNP水平高于对照组(P<0.05),且心功能分级越高,Cys C、BNP水平越高,各组间差异有统计学意义(P<0.05)。研究组LVEF低于对照组(P<0.05),LVEDd、LVM、LVMI高于对照组(P<0.05)。Ⅳ级LVEF低于Ⅱ、Ⅲ级(P<0.05),LVEDd、LVM、LVMI高于Ⅱ、Ⅲ级(P<0.05)。经Pearson检验,患者血清Cys C、BNP水平与LVEF呈负相关(P<0.05),与LVEDd、LVM、LVMI呈正相关(P<0.05)。结论血清Cys C、BNP在老年慢性心衰患者中呈高表达,两者水平与心衰严重程度及心室重构密切相关,是评估心室重构的良好指标,能为心衰患者进行危险分层,可作为临床治疗参考依据。Objective To investigate the correlation between serum cystatin C(Cys C)and B-type brain natriuretic peptide(BNP)expression levels and ventricular remodeling in elderly patients with chronic heart failure.Methods 70 patients with chronic heart failure in The First Affiliated Hospital of Henan Science & Technology University from August 2019 to September 2020 were enrolled as study group,which was further divided into three subgroups according to the NYHA classifications,grade Ⅱ(n=25),grade Ⅲ(n=32),and grade Ⅳ(n=13);meanwhile,another 30 healthy individuals were set as control group.Serum levels of Cys C and BNP as well as ventricular structural and functional indicators in each group were measured and compared,and the correlation was tested by Pearson analysis.Results Serum levels of Cys C and BNP in study group were higher than those in control group(P<0.05).With an increase in cardiac function,the levels of Cys,C and BNP increased gradually(P<0.05).LVEF in study group was lower than that in control group(P<0.05),while LVEDd,LVM and LVMI in study group were higher than those in control group(P<0.05).LVEF of grade Ⅳ was lower than that of grade Ⅱ and Ⅲ(P<0.05),while LVEDd,LVM and LVMI were higher than those of grade Ⅱ and Ⅲ(P<0.05).Pearson test showed that the serum levels of Cys C and BNP were negatively correlated with LVEF(P<0.05),and positively correlated with LVEDd,LVM and LVMI(P<0.05).Conclusion Serum Cys C and BNP are highly expressed in elderly patients with chronic heart failure,and their levels are closely related to the severity of heart failure and ventricular remodeling,which are good indicators for the evaluation of ventricular remodeling and can be used for risk stratification of patients with heart failure and as a reference for clinical treatment.
