摘　　要：目的分析CRRT治疗对急性肾衰竭患者的疗效及血流动力学及炎症因子的影响。方法随机选取医院2018年6月-2019年12月收治的急性肾衰竭患者180例,随机数字法将其分成对照组与观察组,每组90例。对照组患者接受间歇性透析治疗,观察组患者接受CRRT治疗。比较两组患者临床疗效、生化指标、血流动力学及炎性因子水平。结果观察组患者尿量600ml/d时间、APACHE Ⅱ分低于对照组(P<0.05)。治疗前两组患者MAP、CO、SVR水平差异比较无统计学意义(P>0.05),治疗后增加;观察组患者MAP、CO、SVR水平高于对照组(P<0.05)。两组患者PH、BUN、Scr水平差异比较无统计学意义(P>0.05),治疗后降低;观察组患者者PH水平低于对照组(P>0.05),差异比较无统计学意义,BUN、Scr水平低于对照组(P<0.05)。对照组患者CRP、IL-6水平低于观察组(P>0.05),治疗后降低;观察组患者CRP、IL-6低于对照组(P<0.05)。结论 CRRT能明显急性肾衰竭患者血流动力学,提升治疗效果。Objective To investigate the effects of continuous renal replacement therapy(CRRT) on hemodynamics and inflammatory factors in patients with acute renal failure.Methods 180 patients with acute renal failure in our hospital from June 2018 to December 2019 were enrolled,and divided into two groups by random number table methods,each with 90 cases.Control group received intermittent hemodialysis,while observation group received CRRT.Then the clinical efficacy,biochemical indexes,hemodynamics and inflammatory factor levels were compared between groups.Results The urine volume of 600 ml/d and the APACHE Ⅱ score of observation group were lower than those of control group(P<0.05).MAP,CO,SVR had no significant difference between groups before treatment(P>0.05),which were increased in both groups after treatment,and were higher in observation group than in control group(P<0.05).PH,BUN,and Scr levels had no significant difference between groups before treatment(P>0.05),which were increased in both groups after treatment.Observation group had lower PH(P>0.05),BUN,and Scr levels(P<0.05) than those of control group after treatment.CRP and IL-6 levels had no significant difference between groups before treatment(P>0.05),which were decreased in both groups after treatment,and were lower in observation group than in control group(P<0.05).Conclusion CRRT can obviously hemodynamics in patients with acute renal failure and improve the treatment effect.
