术前双氯芬酸钠滴眼液联合术后妥布霉素地塞米松滴眼液对家兔白内障术后炎症反应的影响

摘　　要：目的:探讨术前双氯芬酸钠滴眼液联合术后妥布霉素地塞米松滴眼液对家兔白内障术后炎症反应的影响。方法:新西兰大白兔40只随机分为对照组(术前玻璃酸钠滴眼液点眼+术后玻璃酸钠滴眼液点眼)、双氯芬酸钠组(术前玻璃酸钠滴眼液点眼+术后双氯芬酸钠滴眼液点眼)、妥布霉素地塞米松组(术前玻璃酸钠滴眼液点眼+术后妥布霉素地塞米松滴眼液点眼)、联合组(术前双氯芬酸钠滴眼液点眼+术后妥布霉素地塞米松滴眼液点眼),每组10只。分别记录术后第1天、术后3周、术后6周角膜水肿程度分级评分、前房闪辉分级评分、晶状体后囊膜混浊(PCO)分级评分、房水中前列腺素(PG)水平;实时荧光定量PCR(qRT-PCR)法及蛋白免疫印迹(WB)法测定晶体Toll样受体4(TLR-4)、核因子κB(NF-κB) mRNA和蛋白水平。结果:各组术后第1天角膜水肿分级评分、前房闪辉分级评分、PCO分级评分、房水PG水平相近(P>0.05);术后3周、6周时,其余3组以上各指标显著低于对照组(P<0.05),联合组各指标分别低于双氯芬酸钠组和妥布霉素地塞米松组(P<0.05),双氯芬酸钠组和妥布霉素地塞米松组间差异无统计学意义(P> 0.05);随术后时间的增加,各组角膜水肿分级评分、前房闪辉分级评分、PCO分级评分、房水PG水平呈现出降低趋势,在术后6周时达到最低值(P<0.05)。其余3组晶状体TLR-4、NF-κB mRNA和蛋白水平显著低于对照组(P<0.05),联合组晶状体TLR-4、NF-κB mRNA和蛋白水平分别低于双氯芬酸钠组、妥布霉素地塞米松组(P<0.05),双氯芬酸钠组、妥布霉素地塞米松组晶状体TLR-4、NF-κB mRNA和蛋白水平相近(P>0.05)。结论:术前双氯芬酸钠滴眼液联合术后妥布霉素地塞米松滴眼液对家兔白内障术后炎症反应具有明显抑制作用,其机制可能与术前双氯芬酸钠滴眼液联合术后妥布霉素地塞米松滴眼液可明显抑制家兔白内障术后晶状体TObjective: To explore the effect of preoperative diclofenac sodium eye drops combined with postoperative tobramycin dexamethasone eye drops on postoperative cataract inflammatory response in rabbits. Methods: Forty New Zealand white rabbits were randomly divided into control group( sodium hyaluronate eye drops before operation + sodium hyaluronate eye drops after operation),diclofenac sodium group( sodium hyaluronate eye drops before operation + diclofenac sodium eye drops after operation),tobramycin dexamethasone group( preoperative sodium hyaluronate eye drops + postoperative tobramycin dexamethasone eye drops),combined group( diclofenac sodium eye drops before operation + tobramycin dexamethasone eye drops after operation) with 10 ones in each group. Grading score of corneal edema degree,anterior chamber flash grading score,grading score of posterior capsule opacification( PCO) and prostaglandin( PG) level in aqueous humor were recorded on the first day after surgery,3 weeks after surgery and 6 weeks after surgery. Real-time fluorescence quantitative PCR( qRT-PCR) and Western blot( WB) were used to determine the levels of mRNA and protein of crystal Toll-like receptor 4( TLR-4) and nuclear factor κB( NF-κB). Results: The corneal edema grading score,anterior chamber flash grading score,PCO grading score and aqueous PG level on the first postoperative day in each group were similar( P>0.05);at 3 and 6 weeks postoperatively,the above indicators of the other 3 groups were significantly lower than the control group( P<0.05);the indicators of the combined group were lower than diclofenac sodium group and tobramycin dexamethasone group( P<0.05);there were no significant differences between diclofenac sodium group and tobramycin dexamethasone group( P> 0.05). With the increase of postoperative time,the corneal oedema grade score,anterior chamber flash grade score,PCO grade score and aqueous PG level in each group showed a decreasing trend,and reached the lowest value at 6 weeks after surgery( P<0.05).
