摘  要:目的分析本院弗氏柠檬酸杆菌耐药情况,同时检测产Ampc酶菌株的基因型,为临床提供参考依据。方法收集2015年-2018年189株弗氏柠檬酸杆菌,纸片扩散法检测抗生素的耐药性;利用头孢西丁行改良Hodge法鉴定产Ampc酶菌株,PCR扩增Ampc酶基因。结果本院弗氏柠檬酸杆菌对头孢类抗生素普遍耐药,对亚胺培南和美罗培南敏感性高达98%;189株菌株共鉴定出产Ampc酶菌株47株,PCR结果显示这些菌株的基因型主要为ADC、DHA和ACT-1型,其中ADC基因型占比62.22%,未检出其余基因型。结论本院弗氏柠檬酸杆菌对常用的头孢类耐药,应加强抗生素的规范使用;产Ampc酶菌株比例较高,基因型为ADC、DHA和ACT-1型。Objective To analyze the drug resistance of Citrobacter freundii in this hospital,and to detect the genotype of Ampc-producing strain,providing reference for clinical doctor. Methods A total of 189 strains of Citrobacter freundii were collected between 2015 and 2018,and the antibiotic resistance was detected by paper diffusion method. The modified Hodge method was used to identify the strain of Ampc enzyme and the gene of Ampc was amplified by PCR. Results The drug sensitivity results showed that the resistance of Citrobacter freundii to cephalosporin antibiotics were higher,and the sensitivity to imamipenem and meiluopeinan was as high as 98%. A total of 47 Ampc-producing strains were identified from 189 strains. PCR results showed that the genotypes of these strains were mainly ADC,DHA,and ACT-1,of which ADC genotypes accounted for 62. 22% and the remaining genotypes were not detected. Conclusion The drug resistance of Citrobacter freundii to commonly used cephalospora should be strengthened. The proportion of Ampc-producing strains is higher,and the main genotypes are ADC,DHA,and ACT-1.
