益气化瘀平肝滋肾方联合西医治疗高血压视网膜病变临床观察

摘　　要：目的:观察益气化瘀平肝滋肾方联合西医治疗高血压视网膜病变(HRP)的临床疗效。方法:122例HRP患者随机分为两组各61例。对照组给予西医治疗,观察组在对照组基础上加用益气化瘀平肝滋肾方治疗,两组疗程均为3个月。观察两组疗效与药品不良反应发生情况,比较两组患者治疗前和治疗1个月、3个月后眼底荧光血管造影定量指标[微血管密度数目(MD)、视网膜静脉充盈时间(VFT)、视网膜动脉显影时间(ADT)]、视网膜中央动脉(CRA)、眼动脉(OA)、睫状后动脉(PCA)的舒张末期流速(EDV)及收缩期峰值流速(PSV),以及血清人生长停滞特异性蛋白(GAS6)、色素上皮源性因子(PEDF)、血管内皮生长因子(VEGF)水平的变化。结果:观察组治疗总有效率高于对照组(P<0.05)。治疗3个月后,两组患者的MD和CRA、OA、PCA的EDV、PSV,以及血清PEDF水平均较治疗前和治疗1个月后有明显升高,VAT、ADT和血清GAS6、VEGF水平则有明显降低(P<0.05);且观察组各项指标均显著优于对照组(P<0.05)。两组药品不良反应发生率比较,差异无统计学意义(P>0.05)。结论:益气化瘀平肝滋肾方联合西医治疗HRP疗效显著,能有效改善血管复灌率,增加血流速度,调节血清GAS6、VEGF、PEDF水平,且安全性较好。Objective:To observe the clinical efficacy of Yiqi Huayu Pinggan Zishen Recipe combined with western medicine in the treatment of hypertensive retinopathy(HRP). Methods:One hundred and twenty-two HRP patients were randomly divided into two groups,with 61 cases in each. The control group was treated with western medicine,and the observation group was treated with Yiqi Huayu Pinggan Zishen Recipe based on the control group,and both were treated for 3 months. The curative effect and adverse drug reactions of the two groups were observed. The quantitative indicators of fundus fluorescence angiography [microvessel density(MD),retinal vein filling time(VFT),and retinal artery development time(ADT)],the end-diastolic velocity(EDV) and peak systolic velocity(PSV) of the central retinal artery(CRA),ophthalmic artery(OA),posterior ciliary artery(PCA),serum human growth arrest specific protein(GAS6),pigment epithelial-derived factor(PEDF),and vascular endothelial growth factor(VEGF) before,one month,and three months after treatment between the two groups were compared. Results:The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05). After three months of treatment,the MD,CRA,OA,PCA EDV,PSV,and serum PEDF levels of the two groups of patients were significantly higher than before and after one month of treatment;VAT,ADT,serum GAS6 and VEGF levels were significantly reduced(P<0.05). And all the indicators of the observation group were significantly better than those of the control group(P<0.05). There was no significant difference in the incidence of adverse drug reactions between the two groups(P>0.05). Conclusion:Yiqi Huayu Pinggan Zishen Recipe combined with western medicine has a significant effect on HRP,which can effectively improve the reperfusion rate of blood vessels,increase the blood flow rate,regulate the levels of GAS6,VEGF and PEDF,and has higher safety.
