达托霉素不良反应89例文献病例分析

摘　　要：目的:探讨达托霉素相关不良反应(ADR)的一般规律和特点,为临床安全用药提供参考。方法:检索文献数据库,搜集达托霉素相关ADR病例资料,按照患者一般情况、用药情况、ADR发生及转归等进行统计分析。结果:共收集60篇文献,89例达托霉素相关ADR病例。ADR关联性评价结果为"很可能"的占比76.40%(68/89);患者男女比例为2.36∶1,中老年患者占比最高(37例,41.57%);用于骨关节感染占比最高为47.17%(50/106);给药方式方法合理的共68例。ADR发生时间在1~2周的最多,占比25.84%(23/89);临床表现以呼吸系统损害(32.13%)为主;严重ADR共26例(29.21%),其中2例导致死亡,1例导致永久性听力降低。结论:达托霉素相关ADR表现多样,应引起医药护人员重视,确保患者用药安全。Objective:To explore the general rules and characteristics of daptomycin-induced adverse reactions,and to provide a reference for clinical safe drug use. Methods:Literature database was searched to collect datomicin-related ADR case data,and statistical analysis was conducted according to patients’ general situation,medication status,ADR occurrence and outcome,etc.Results:A total of 60 articles,89 cases of daptomycin-induced ADRs was enrolled in this study. In these cases,the evaluation result was "possibility" to account for the total 76.40%(68/89),male to female ratio was 2.36∶1,most of the ADR occurred in middle and elderly aged-patients,was 37(41.57%) cases respectively. The highest proportion for bone and joint infection was 47.17%(50/106),reasonable administration method was 68 cases. Most ADRs occurred within 1-2 weeks,which accouts for 25.84%(23/89). Clinical manifestations was mainly respiratory system disorders(32.13%). Serious ADRs were 26 cases(29.21%),2 cases resulted in death,one case resulted in permanent hearing loss. Conclusion:Daptomycin-induced ADRs was varied. It should be paid attention to by medical and nursing personnel to ensure the safety of medication.
