针刺配合中药湿敷治疗糖尿病周围神经病变的临床观察

摘　　要：目的通过观察针刺配合中药湿敷治疗糖尿病周围神经病变的临床疗效,以期为该病治疗提供一种有效的治疗疗法。方法2018年1月—2019年6月,将60例符合纳入标准的DPN患者,采用随机数字表法分为对照组和观察组各30例。在基础治疗的同时,对照组口服甲钴胺片,0.5 mg TID;观察组以针刺配合中药湿敷,QD:2w为1疗程,共治疗2个疗程。采用中医症候量化积分、Toronto评分对两组患者治疗前后进行评估,将结果进行统计学分析。结果观察组总有效率为83.33%,对照组56.67%;中医症候量化、压力觉积分治疗前后及组间比较,差异具体统计学意义(P<0.05);膝反射、踝反射积分变化治疗前后及组间比较,差异无统计学意义(P>0.05)。结论针刺配合中药湿敷治疗DPN可明显改善临床症状,尤其对于肢体麻木、发凉、无力、疼痛等中医症候改善尤为显著,对压力觉改善较为明显,具有良好的临床疗效。Objective To observe the clinical efficacy of acupuncture combined with Chinese medicine wet compress in the treatment of diabetic peripheral neuropathy,in order to provide an effective treatment for the disease.Methods 60 patients with DPN who met the inclusion criteria were randomly divided into 30 patients in the control group and 30 patients in the observation group according to the random number table method.At the same time of basic treatment,the control group was given oral cobalamin tablet(mikebo)0.5 mg TID.The observation group was treated with acupuncture combined with wet compress of traditional Chinese medicine QD.All the above methods were 2 weeks for one course of treatment,with a total of 2 courses of treatment.TCM syndrome quantitative score and Toronto score were used to evaluate the patients in the two groups before and after treatment,and the results were statistically analyzed.Results The total effective rate was 83.33%in the observation group and 56.67%in the control group.There were significant differences before and after TCM syndrome quantification and pressure perception score treatment(P<0.05).There was no significant difference in knee and ankle reflexes before and after treatment(P>0.05).Conclusion Acupuncture combined with Chinese medicine wet compress can significantly improve clinical symptoms of DPN,especially for limb numbness,coolness,weakness,pain and other symptoms of Chinese medicine,the improvement of pressure sensation is relatively obvious,has a good clinical efficacy,worthy of promotion and application in clinical.
