摘 要:目的探讨2型糖尿病(T2DM)患者外周血可溶性白介素-2(sIL-2R)水平与糖尿病肾病(DN)的相关性。方法选取2018年9月至2019年7月在我院内分泌科收治的T2DM患者327例,其中正常蛋白尿(NA)组190例、微量白蛋白尿(MA)组87例、大量白蛋白尿(OA)组50例。检测并比较三组患者年龄、性别、体质量指数(BMI)、糖化血红蛋白(HbA1c)、病程、外周血sIL-2R、血肌酐、肾小球滤过率(eGFR)、尿白蛋白/肌酐比值(UACR)水平差异。对比三组患者外周血可溶性白介素-2(sIL-2R)的水平差异,研究外周血可溶性白介素-2(sIL-2R)对糖尿病肾病的影响。结果血清sIL-2R在NA组、MA组及OA组中水平依次升高,组间比较及组内两两比较,差异均有统计学意义(P<0.05或P<O.01);多元逐步回归分析显示,病程、sIL-2R是ACR的独立影响因素(P<0.01)。结论外周血sIL-2R水平是DN的独立影响因素,进一步证实细胞免疫可能参与DN的发生和发展。Objective To explore the correlation between soluble interleukin-2(sIL-2R)level in peripheral blood and diabetic nephropathy(DN)in the patients with Type 2 diabetes mellitus(T2DM).Methods A total of 327 patients with T2DM admitted to and treated in Department of Endocrinology of our hospital from September 2018 to July 2019 were selected,including 190 patients in the normal albuminuria(NA)group,87 patients in the microalbuminuria(MA)group and 50 patients in the massive albuminuria(OA)group.The differences among the three groups of patients were examined through the comparison of age,sex,body mass index(BMI),glycosylated hemoglobin(HbA1c),course of disease,sIL-2R in peripheral blood,serum creatinine,glomerular filtration rate(GFR),urinary albumin/creatinine ratio(UACR)levels.The levels of soluble interleukin-2(sIL-2R)in peripheral blood of the three groups of patients were compared for the study of the effect of soluble interleukin-2(sIL-2R)in peripheral blood on DN.Results The level of serum sIL-2R in the NA group,MA group and OA group increased in turn,and the differences were all significant no matter among the groups or within each group(P<0.05 or P<0.01).Multiple stepwise regression analysis showed that the course of disease and sIL-2R were independent influencing factors of ACR(P<0.01).Conclusion The level of sIL-2R in peripheral blood is an independent influencing factor of DN,which further confirms that cellular immunity may participate in the occurrence and development of DN.
