摘 要:目的考察非对称回波的最小二程估算法迭代水脂分离(IDEAL-IQ)技术与氢质子磁共振波谱(1H-MRS)定量评估急性非胆源性胰腺炎脂肪含量的可行性。方法回顾性选取2017年1月至2019年11月于湖州市第一人民医院就诊的急性非胆源性胰腺炎患者90例及同期健康志愿者90例,采用IDEAL-IQ与1H-MRS定量测量急性非胆源性胰腺炎的脂肪分数,分析并比较采用IDEAL-IQ和1H-MRS两种方式测量所得脂肪含量的差异及相关性。结果两组扫描成功率比较,IDEAL-IQ较1H-MRS扫描成功率更高(97.78%vs 84.44%),差异有统计学意义(P<0.05)。两组内胰腺各部位脂肪含量比较,差异均无统计学意义(P>0.05)。两组水峰下面积、脂峰下面积、RLC和FF值比较,差异均有统计学意义(P<0.05)。Spearman相关性分析显示,IDEAL-IQ与1H-MRS测量的胰腺脂肪含量呈正相关(r=0.698,P=0.001)。结论1H-MRS和IDEAL-IQ均可定量测量胰腺的脂肪含量,两者测量的急性非胆源性胰腺炎脂肪含量呈正相关。Objective To investigate the feasibility of quantitative evaluation of the fat content of acute non-biliary pancreatitis by the least square estimation iterative water-fat separation technique(IDEAL-IQ)of asymmetric echo and hydrogen proton magnetic resonance spectroscopy(1H-MRS).Methods A total of 90 patients with acute non-biliary pancreatitis in Huzhou First People′s hospital and 90 healthy volunteers collected during the same period of time from January 2017 to November 2019 were selected.The fat fraction of acute non-biliary pancreatitis was quantitatively measured by IDEAL-IQ and 1H-MRS methods.The difference and correlation of fat content measured by IDEAL-IQ and 1H-MRS were analyzed and compared.Results The scan success rate was compared between the two groups of subjects,and the scan success rate of IDEAL-IQ was higher than that of 1H-MRS scan(97.78%vs 84.44%).The difference was statistically significant(P<0.05).There was no statistically significant difference in the fat content of each part of pancreas in the two groups(P>0.05).There were statistically significant differences in the area under the water peak,the area under the fat peak,RLC and FF values between the two groups(P<0.05).Spearman correlation analysis showed that IDEAL-IQ was positively correlated with pancreatic fat content measured by 1H-MRS(r=0.698,P=0.001).Conclusion Both 1H-MRS and IDEAL-IQ can quantitatively measure the fat content of the pancreas.The fat content of acute non-biliary pancreatitis measured by the two methods is linearly correlated.
